摘　　要：目的探讨2017—2021年海安市三级医院慢性乙型病毒性肝炎(chronic hepatitis B,CHB)患者HBV基因型和亚型的分布状况。方法选取2017年1月—2021年12月经海安市三级医院确诊的500例CHB患者,收集血清样本检测丙氨酸氨基转移酶(ALT),PCR检测HBV DNA。计量资料行t检验;占比计数资料采用χ^(2)检验。结果HBeAg阳性患者337例,阴性163例;HBV基因型分布,70.80%的C基因型(C1亚型5例,C2亚型340例,C1/C2混合亚型9例),20.60%的B基因型(均为Ba亚型),8.60%的B/C基因型。HBeAg阳性组与HBeAg阴性组CHB患者的HBV C基因型和B基因型差异无统计学意义(χ^(2)=1.356,P=0.244),两组患者HBV B/C混合感染的基因型均为Ba和C2亚型;HBV B基因、C基因型感染患者的HBV DNA水平与HBV B/C基因型混合感染患者相比,差异无统计学意义(t=0.815、1.178,P>0.05)。结论2017—2021年海安市三级医院CHB患者B基因型、C基因型HBV感染占比较高,HBV亚型以Ba和C2亚型感染比例较高,临床需加强防控管理。Objective To investigate the distribution of Hepatitis B virus(HBV)genotypes and subtypes in patients with chronic hepatitis B(CHB)in ClassⅢhospitals in Hai'an from 2017 to 2021.Methods A total of 500 CHB patients diagnosed at Hai'an tertiary hospital from January 2017 to December 2021 were selected.Serum samples were collected to detect alanine aminotransferase(ALT),and PCR was used for HBV DNA detection.Measurement data were analyzed using the t test,and proportion counting data were assessed with theχ^(2) test.Results Three hundred and thirty-seven patients were HBeAg positive,and 163 were HBeAg negative.The distribution of HBV genotypes was as follows:70.80%C genotype(5 C1 subtypes,340 C2 subtypes,9 C1/C2 mixed subtypes),20.60%B genotype(all Ba subtypes),and 8.60%B/C genotype.There was no statistically significant difference in HBV C genotype and B genotype between the HBeAg-positive and HBeAg-negative groups in CHB patients(χ^(2)=1.356,P=0.244).Co-infection with HBV B/C genotype in both groups was mainly Ba and C2 subtypes.There was no statistically significantdifference between the HBV DNA levels in patients infected with HBV B gene and C genotype and those in patients with HBV B/C genotype mixed infection(t=0.815,1.178,P>0.05).Conclusion From 2017 to 2021,CHB patients in Hai'an tertiary hospital showed a relatively high proportion of HBV infection with B and C genotypes,with a higher infection rate of Ba and C2 subtypes.Clinical prevention and control management should be strengthened.
