益气清利化瘀方治疗原发性IgA肾病

摘　　要：目的探讨益气清利化瘀方对脾肾气虚兼湿瘀证IgA肾病患者症状改善和炎性因子水平的影响。方法选取2020年3月—2021年3月在我院接受治疗的264例脾肾气虚兼湿瘀证原发性IgA肾病患者作为研究对象,依据治疗方法不同分为对照组与治疗组,各132例。对照组予以氯沙坦钾治疗,治疗组在对照组基础上予以益气清利化瘀方治疗。分析益气清利化瘀方对该证型肾病患者中医症状改善状况、肾功能及炎症因子变化情况,以及治疗的有效性和安全性分析。结果治疗后,2组尿红细胞数、24 h-UTP、Scr、BUN、UA水平均降低;与对照组比较,益气清利化瘀方组降低幅度更显著(P<0.05);治疗后,2组中医证候积分均降低,益气清利化瘀方组积分低于对照组(P<0.05);益气清利化瘀方组治疗有效率(95.45%)高于对照组(68.18%),治疗后,2组hs-CRP、TNF-α、IL-2、IL-4、IL-5、IL-6水平降低,IFN-γ、IL-10水平升高;与对照组比较,益气清利化瘀方组变化幅度更显著(P<0.05);组间比较,差异具有统计学意义(P<0.05)。结论益气清利化瘀方对脾肾气虚兼湿瘀证原发性IgA肾病患者的中医症状、尿红细胞计数及肾功能改善作用明显,可降低炎性反应,安全性可靠。Objective To explore the effects of Yiqi Qingli Huayu Recipe on the improvement of symptoms and the levels of infammatory factors in the treatment of the syndrome of qi deficiency of spleen and kidney combined with dampness and stagnation in patients with primary IgA nephropathy.Methods A total of 264 patients with primary IgA nephropathy of the syndrome of qi deficiency of spleen and kidney combined with dampness and stagnation who were treated in our hospital from March 2020 to March 2021 were selected as the study subjects.According to the different treatment methods,they were divided into two groups:the control group and the treatment group,with 132 cases in each group.The control group was treated with losartan potassium,while the treatment group was treated with Yiqi Qingli Huayu Recipe on the basis of the control group.The safety and effectiveness of treatment,improvement of traditional Chinese medicine symptoms,changes in renal functions and inflammatory factors were analyzed.Results After treatment,the levels of urine red blood cell count,24-hour UTP,Scr,BUN,and UA were decreased in both groups,and the decrease was more significant in the treatment group(P<0.05).After treatment,the scores of traditional Chinese medicine symptoms were decreased in both groups,and the scores were lower in the treatment group than those in the control group(P<0.05).The effective rate of the treatment group(95.45%)was higher than that of the control group(68.18%)(P<0.05).After treatment,the levels of hs-CRP,TNF-α,IL-2,IL-4,IL-5,and IL-6 were decreased in both groups,while the levels of IFN-,IL-10 were increased,and the range of changes was more significant in the treatment group(P<0.05).Conclusion Yiqi Qingli Huayu Recipe can significantly improve the TCM symptoms,urinary red blood cell count and renal functions of patients with primary IgA nephropathy of the syndrome of qi deficiency of spleen and kidney combined with dampness and stagnation,reducing inflammatory reactions and having a reliability and safety.
