张春馀温阳益气通络法治疗老年脱疽经验  

ZHANG Chunyu's Experience in Treating Elderly Gangrene by Wenyang Yiqi Tongluo Method

摘　　要：脱疽是多种疾病导致的以肢体末端缺血坏疽为主的严重病症,好发于老年人。张春馀认为老年人阳气虚衰、气血不旺,血脉易于瘀阻,加之年老肾阳不足,温煦无力,易受寒冷刺激而好发本病。张春馀针对其病因病机运用温阳益气通络法治疗老年脱疽,临床常采用阳和汤、当归四逆汤等方剂,并喜加用淡附片、吴茱萸等温热助阳药,对局部溃烂疮面还加用化腐生肌外治法促进愈合,取得了良好的临床效果。Gangrene is a serious symptom caused by a variety of diseases characterized by extremities ischemic,which mainly occurs in the elderly.ZHANG Chunyu believes that the elderly Yang Qi deficiency,Qi and blood is not flourishing,vascellum are easy to be blocked,in addition to the deficiency of kidney Yang,unable to warm,easy to be stimulated by cold,and prone to this disease.According to its etiology and pathogenesis,ZHANG Chunyu used Wenyang Yiqi Tongluo method to treat elderly gangrene.He often used Yanghe Decoction,Danggui Sini Decoction and other prescriptions,and liked to add the warm Yang drugs such as Danfu tablet and Evodia officinalis.He also used external treatment methods such as transforming putrefaction muscle to promote healing of local ulcerative ulcers,has achieved good clinical results.
