火针点刺治疗老年人腓肠肌痉挛的随机对照研究

摘　　要：目的:探讨火针点刺治疗老年人夜间腓肠肌痉挛的临床疗效。方法:研究纳入80例老年腓肠肌痉挛患者,随机分为观察组(n=39)和对照组(n=38)。观察组采用火针点刺治疗,对照组采用Park针治疗。2组均每周治疗3次,3次为1个疗程,共治疗3个疗程。比较2组患者症状、体征以及失眠改善情况。结果:观察组总有效率为97.44%,对照组总有效率为57.89%,2组比较差异有明显统计学意义(P<0.001)。2组治疗1周、2周、3周后的症状分级积分、体征分级积分、失眠严重指数、炎症细胞因子、氧化应激等指标均较治疗前及前一治疗时间点改善(P<0.05),且观察组上述指标的改善程度均优于对照组(P<0.01)。结论:火针点刺治疗能有效缓解老年人夜间腓肠肌痉挛状态,改善睡眠,提高抗氧化应激能力,减轻炎症损伤,可以在临床上广泛推广。Objective:To explore the clinical efficacy of fire needle pricking in the treatment of nocturnal gastrocnemius spasm in the elderly.Methods:80 elderly patients with gastrocnemius spasm were included in the study and randomly divided into observation group(n=39)and control group(n=38).The observation group was treated with fire needle pricking,and the control group was treated with Park needle.Both groups were treated 3 times a week,and 3 times were 1 course of treatment,for a total of 3 courses of treatment.The improvement of symptoms,signs and insomnia were compared between the two groups.Results:The total effective rate was 97.44%in the observation group,while 57.89%in the control group.There was significant difference between the two groups(P<0.001).The symptom grading score,physical sign grading score,insomnia severity index,inflammatory cytokines and oxidative stress in the two groups after 1 week,2 weeks and 3 weeks of treatment were better than those before treatment and the time point before treatment(P<0.05).The degree of improvement of the above indexes in the observation group was better than that in the control group(P<0.01).Conclusion:Fire needle pricking treatment can effectively alleviate nocturnal gastrocnemius muscle spasms in elderly patients,optimize sleep quality,improve antioxidant stress capacity,reduce inflammatory damage,and can be widely used in clinical application.
