益心饮治疗老年冠心病合并心律失常的临床疗效观察

摘　　要：目的 探讨益心饮用于老年冠心病合并心律失常治疗的临床效果。方法 选取2022年11月至2023年5月就诊于海军军医大学第二附属医院中医科的老年冠心病合并心律失常的患者90例为研究对象。以随机法分为对照组(n=45)和试验组(n=45),对照组行常规西药治疗,试验组在对照组基础上结合复方中药益心饮治疗,分析两组临床症状积分、心功能改善以及不良反应发生率。结果 治疗前,两组患者气短、心悸、乏力等症状积分,室性期前收缩、房性期前收缩及房室交界性期前收缩发生的次数,心排血量、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)及6 min步行距离比较,差异无统计学意义(P>0.05)。治疗后,与对照组比较,试验组各症状积分,室性期前收缩、房性期前收缩及房室交界性期前收缩发生的次数和LVESD均降低(P<0.05),心排血量、LVEF和6 min步行距离则升高(P<0.05),且试验组疗效优于对照组(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论 益心饮联合西药常规治疗,可改善冠心病心律失常患者的临床症状,在改善心律失常及心功能方面效果明显,疗效优于单独使用西药治疗。Objective To investigate the clinical effect of Yixin Yin in the treatment of coronary heart disease complicated with arrhythmia in the aged.Methods A total of 90 elderly patients with coronary heart disease and arrhythmia admitted to the Department of Traditional Chinese Medicine of Changzheng Hospital from November 2022 to May 2023 were selected as the study objects.They were randomly divided into control group(n=45)and experimental group(n=45).The former was treated with conventional western medicine,while the latter was treated with conventional western medicine and Yixin Yin.The clinical syndrome integral,improvement of cardiac function and incidence of adverse drug reactions(ADRs)were statistically analyzed.Results Before treatment,there was no significant difference in clinical syndrome integral of symptoms such as shortness of breath,palpitation,and fatigue,the frequency of ventricular,atrioventricular,and atrioventricular junction premature contraction,cardiac output,left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF)and 6 min walking distance between the two groups(P>0.05).After treatment,the clinical syndrome integral,the frequency of ventricular,atrioventricular,and atrioventricular junction premature contraction and LVESD in both groups were significantly decreased(P<0.05),while the cardiac output,LVEF and 6 min walking distance in both groups were significantly increased(P<0.05).And the therapeutic effect in the experimental group was better than that in the control group(P<0.05).There was no significant difference in the incidence of ADRs between the two groups(P>0.05).Conclusion Yixin Yin combined with conventional western medicine can significantly improve the clinical symptoms,arrhythmia and cardiac function of elderly patients with coronary heart disease complicated with arrhythmia,which is better than conventional western medicine alone.
