2016—2022年宿迁地区医疗机构用血退回情况分析

摘　　要：目的通过分析宿迁市2016—2022年临床用血机构退血原因,探寻改进措施,确保产品质量,提高服务水平以降低退血率。方法收集“穿越输血标准化系统”中宿迁市2016—2022年血液供应情况,对宿迁市医疗机构退血的种类、数量、原因,以及退回血液的供血者情况进行统计分析。结果2016—2022年宿迁市中心血站发往临床用血机构红细胞类和血浆类血液制品共计771414.3 U,退血率为0.097‰,其中,红细胞类血液制品退回率为0.112‰(45.5/406178.5),退回原因主要为配血不合,其中72.84%为不规则抗体筛查阳性,27.16%为直抗阳性;血浆类血液制品退回率为0.08‰(29.25/365235.8),退回原因主要为絮状物、脂肪血和破损。血液制品退回率在不同年龄组供血者间差异有统计学意义(P<0.05),以35~45岁供血者占比最高(40.48%)。结论2016—2022年宿迁地区医疗机构退血原因主要是配血不合、絮状物以及脂肪血等,需要按规定条件储存、运输、融化血浆,以及加强供血者献血前的筛查。Objective To analyze the causes of blood withdrawal in clinical blood institutions in Suqian City from 2016 to 2022,and to explore improvement measures to ensure product quality and improve service level to reduce blood withdrawal rate.Methods The blood supply in Suqian City from 2016 to 2022 was collected from the standardized blood transfusion system,and the types,quantities,and causes of blood withdrawal in medical institutions in Suqian City were statistically analyzed.Results From 2016 to 2022,a total of 771414.3 U of erythrocyte and plasma blood products were sent to clinical blood institutions from Suqian Central Blood Station,and the blood withdrawal rate was 0.097‰.Among them,the return rate of erythrocyte blood products was 0.112‰(45.5/406178.5),and the main cause for the return was mismatch of blood,of which 72.84%were positive for irregular antibody screening,and 27.16%were positive for direct antibody.The return rate of plasma blood products was 0.08‰(29.25/365235.8),and the main reasons for return were flocculent,fat blood,and damage.There was a significant difference in the return rate of blood products among blood donors in different age groups(P<0.05),and the proportion of blood donors aged 35-45 was the highest(40.48%).Conclusion From 2016 to 2022,the main reasons for blood withdrawal in medical institutions in Suqian area are incompatible blood matching,flocculent and age factors fat blood,and it is necessary to store,transport,and thaw plasma according to the prescribed conditions,and strengthen the screening of blood donors before blood donation.
