摘 要:目的通过检测结核性渗出性胸膜炎患者的血清超敏C反应蛋白/前白蛋白比值(hs-CRP/PAB)、D-二聚体(D-D),研究hs-CRP/PAB、D-D在结核性渗出性胸膜炎中表达水平及其诊断价值。方法110例经胸腔镜确诊的胸腔积液患者,根据病理活检诊断结果分为结核性渗出性胸腔积液组(60例)和非结核性胸腔积液组(50例)作为对照组。采集两组患者入院后的静脉血,检测其血清hs-CRP/PAB、D-D水平。采用多因素Logistic回归分析影响结核性渗出性胸膜炎发生的危险因素。采用受试者工作特征(ROC)曲线分析血清hs-CRP/PAB、D-D水平对结核性渗出性胸膜炎的诊断价值。结果结核性渗出性胸腔积液组血清hs-CRP/PAB、D-D表达水平均高于对照组(P<0.05),多因素Logistic回归分析显示,hs-CRP/PAB高水平、D-D高水平是影响结核性渗出性胸膜炎发生的危险因素(P<0.05)。hs-CRP/PAB、D-D联合诊断结核性渗出性胸膜炎的价值高于hs-CRP/PAB及D-D单独诊断。结论hs-CRP/PAB、D-D在结核性渗出性胸膜炎血清中表达水平均升高,两者是影响结核性渗出性胸膜炎发生的危险因素,两者联合检测对于结核性渗出性胸膜炎诊断具有较高的临床价值。【Objective】To study the expression level and diagnostic value of hypersensitive C-reactive protein/prealbumin ratio(hs-CRP/PAB)and D-dimer(D-D)in tuberculous exudative pleurisy by detecting the serum hs-CRP/PAB and D-D in patients with tuberculous exudative pleurisy.【Methods】According to the results of pathological biopsy,110 patients with pleural effusion diagnosed by thoracoscopy were divided into tuberculous exudative pleural effusion group(60 cases)and non-tuberculous pleural effusion group(50 cases)as the control group.The venous blood of the two groups of patients after admission was collected to detect the serum levels of hs-CRP/PAB and D-D.Multivariate logistic regression analysis was used to analyze the risk factors of tuberculous exudative pleurisy.The receiver operating characteristic curve was used to analyze the diagnostic value of serum hs-CRP/PAB and D-D levels in tuberculous exudative pleurisy.【Results】The expression levels of serum hs-CRP/PAB and D-D in the tuberculosis group were significantly higher than those in the control group(P<0.05).Multivariate logistic regression analysis showed that high levels of hs-CRP/PAB and high levels of D-D were risk factors for tuberculous exudative pleurisy(P<0.05).The value of hs-CRP/PAB and D-D combined diagnosis of tuberculous exudative pleurisy is higher than that of hs-CRP/PAB and D-D alone.【Conclusion】The expression levels of hs-CRP/PAB and D-D in the serum of tuberculous exudative pleurisy are increased,and both are risk factors for the occurrence of tuberculous exudative pleurisy.The combined detection of the two has a high clinical value for the diagnosis of tuberculous exudative pleurisy.
