摘　　要：目的探讨三相骨显像半定量分析法在鉴别恶性骨肿瘤术后的复发与感染中的临床价值。方法研究纳入在恶性骨肿瘤术后行三相骨显像半定量分析法的29人次患者,进行回顾性分析。由经验丰富的2名核医学医师通过三相骨显像视觉分析法及半定量分析法进行独立、盲法阅片。获得三相骨显像各个时相的半定量分析指标的数值。应用受试者工作特征曲线(ROC曲线)和t检验进行统计学分析。结果三相骨显像视觉分析法鉴别恶性骨肿瘤患者术后的局部复发与骨髓感染的诊断特异性较低(27.27%)。三相骨半定量分析法中,患侧平均放射性计数和放射性计数率-时间曲线斜率的诊断准确率最高。当所测血流相平均放射性计数>2.9或血流相放射性计数率-时间曲线斜率>2.95时,考虑病变为复发可能大。结论三相骨显像半定量分析法可以更加精准的反映病灶部位的血流灌注情况,对早期发现恶性骨肿瘤患者术后复发有重要的作用。Objective To explore the clinical value of three phase bone scintigraphy semi-quantitative analysis in differentiating recurrence and infection after malignant bone tumor operation.Methods A retrospective analysis was performed on 29 patients who underwent three phase bone scintigraphy semi-quantitative analysis after operation of malignant bone tumors.Two experienced nuclear medicine physicians independently and blindly read the films by three-phase bone imaging visual analysis and semi-quantitative analysis.To obtain the semi-quantitative indexes of three-phase bone imaging.Use the ROC curve and t test to statistical analysis.Results The specificity of three phase bone scintigraphy in the diagnosis of postoperative recurrence and infection was low(27.27%).The diagnostic accuracy of the average radioactivity count of the affected side in blood flow and the slope of the radioactivity counting rate-time curve slope was the highest in the semi-quantitative analysis of three phase bone scintigraphy.When the average radioactivity count of blood flow was larger than 2.9 or the slope rate of radioactivity counting rate-time curve was more than 2.95,it was possible to consider the lesion for recurrence.Conclusion The semi-quantitative analysis of three-phase bone imaging can more accurately reflect the blood perfusion situation of the lesion site,which plays an important role in the early detection of postoperative recurrence of malignant bone tumors.
