摘 要:目的应用微血流成像(SMI)技术评价肾脏小叶间动脉血流显示状况,探讨其对早期肾功能不全者的应用价值。方法选取拟行肾活检、无恶性疾病、未进行血液透析的肾功能不全患者90例为观察组,选择同年龄段、体脂率相近的健康成年人90例为对照组;收集观察组实验室指标、病理分型,在SMI模式下观察所有研究对象同一部位肾脏小叶间动脉血流显示情况,并对SMI模式下肾脏小叶间动脉血流显示条数进行计数。结果SMI显示肾脏小叶间动脉血流显示条数观察组较对照组减少,两组间比较差异存在统计学意义(t=12.71,P<0.001);观察组中血检尿酸、肌酐、尿素氮(-)与尿酸、肌酐、尿素氮(+)者肾脏小叶间动脉血流显示条数差异存在统计学意义(t=2.36,P=0.021),指标阳性者较阴性者血流分布条数减少;随着尿酸、肌酐、尿素氮数值升高,肾脏小叶间动脉血流显示进一步减少;早期肾功能不全不同病理分型血流显示条数差异无统计学意义(F=1.39,P=0.25)。结论应用SMI技术半定量分析可为临床提供早期肾功能不全患者肾脏微循环受损的佐证依据。Objective To evaluate the display status of visceral interlobular artery blood flow using superb microvascular imaging(SMI)and explore its application value in early renal insufficiency patients.Methods A total of 90 patients with renal insufficiency who were admitted and planned to undergo renal biopsy,without malignant diseases,and without undergoing hemodialysis were selected as the observation group.90 healthy adults of the same age group with similar body fat rates were selected as the control group;number laboratory indicators and pathological subtypes from the observation group,observed the display of renal interlobular artery blood flow in the same area of all study subjects under SMI mode,and counted the of renal interlobular artery blood flow under SMI mode.Results SMI showed a decrease in the number of renal interlobular artery blood flow in the observation group compared to the control group,and there was a statistical difference between the two groups(t=12.71,P<0.001);There was a statistically significant difference(t=2.36,P=0.021)in the number of renal interlobular artery blood flow indicators between blood tests for uric acid,creatinine,and urea nitrogen(-)and uric acid,creatinine,and urea nitrogen(+)in the observation group.The number of blood flow indicators decreased in patients with positive indicators compared to those with negative indicators;As the values of uric acid,creatinine,and urea nitrogen increase,the renal interlobular artery blood flow showed further sparsity;There was no statistically significant difference in the number of blood flow indicators among different pathological subtypes of early renal insufficiency(F=1.39,P=0.25).Conclusions The application of SMI semi quantitative analysis can provide clinical evidence for early renal insufficiency patients with impaired renal microcirculation.
