摘 要:目的探讨超低速血流成像(SlowFlowHD)技术检测正常胎儿胎盘微血流灌注临床应用的可行性。方法选取妊娠20~36周的正常单胎孕妇318例,用常规彩色多普勒血流成像(CDFI)和SlowFlowHD技术检测胎盘内血管结构情况,比较两者显示微血管结构的差异性;使用三维能量多普勒超声显示并通过VOCAL软件脱机分析,测定胎盘微血管血流参数:血管指数(VI)、血流指数(FI)、血管血流指数(VFI),分析VI、FI、VFI随孕周的变化趋势。结果SlowFlowHD技术对胎盘三级血管的显示率为95.91%,一、二级血管全部显示;CDFI对一、二级血管部分显示,三级血管显示不清。VI、FI、VFI随孕周的增加呈增大趋势。结论SlowFlowHD技术可以显示胎盘三级微血管结构,实现血管绒毛颈和绒毛叶水平的显示。VI、FI、VFI提供了微血管的量化指标并与孕周正相关,该技术可用于评价胎盘微血流灌注。Objective To explore the feasibility of ultra-low velocity flow imaging(SlowFlowHD)in detecting microflow perfusion of normal fetal placenta.Methods A total of 318 normal singleton pregnant woman at 20-36 weeks pregnancy were selected,and the placental vascular structure was detected using conventional color Doppler flow imaging(CDFI)and SlowFlowHD.The differences in microvascular structure between the two were compared;Using three-dimensional energy Doppler ultrasound display and offline analysis using VOCAL software,the placental microvascular vascularization index(VI),flow index(FI),and vascularization flow index(VFI)were measured to analyze the trend of VI,FI,and VFI with gestational age.Results The display rate of placental tertiary vessels by SlowFlowHD was 95.91%,and all the primary and secondary vessels were displayed.CDFI showed part of the primary and secondary vessels,but the tertiary vessels were not clearly displayed.VI,FI and VFI increased with the increase of gestational weeks.Conclusions SlowFlowHD can display the tertiary microvascular structure of placenta and realize the display of vascular villous neck and villous leaf level.VI,FI and VFI provide quantitative indicators of microvessels and have a positive correlation with gestational weeks.This technique can be used to evaluate placental microvascular perfusion.
