经阴道三维超声Omniview成像技术对宫腔粘连的诊断价值分析

摘　　要：目的探讨分析经阴道三维超声自由解剖(Omniview)成像技术在诊断宫腔粘连(IUA)方面的临床应用价值。方法回顾性分析106例疑似IUA患者。对所有患者进行经阴道三维超声Omniview技术和传统三维渲染(Render)技术检查以及宫腔镜检查,并以宫腔镜检查结果为诊断IUA的金标准。使用Kappa检验比较分析两种超声成像技术与金标准检查结果的一致性,并绘制受试者工作特征(ROC)曲线以评估其诊断效能。结果Omniview成像技术与金标准诊断结果的一致性良好(Kappa=0.865),Render成像技术与金标准诊断结果的一致性一般(Kappa=0.521);Omniview成像技术诊断IUA的灵敏度、特异度、准确度及约登指数均显著高于Render技术。ROC曲线提示,Omniview技术诊断IUA的曲线下面积(AUC)为0.927(95%CI:0.849~1.000),显著>Render技术诊断IUA的AUC为0.816(95%CI:0.728~0.935)。Omniview技术诊断轻、中、重度IUA的灵敏度、特异度、准确度及约登指数均高于Render技术。结论相较于传统的Render成像技术,Omniview技术对IUA的诊断价值更高。Objective To investigate the clinical application value of Omniview in the diagnosis of intrauterine adhesions(IUA).Methods 106 patients with suspected IUA were retrospectively analyzed.All patients were examined by transvaginal three-dimensional ultrasound Omniview and traditional three-dimensional rendering(Render)as well as hysteroscopy,and the results of hysteroscopy were used as the gold standard for diagnosing IUA.Kappa test was used to compare and analyze the consistency between the two ultrasound imaging techniques and the gold standard test results,and the receiver operating characteristic(ROC)curve was drawn to evaluate its diagnostic efficiency.Results Omniview had a good consistency with gold standard diagnosis results(Kappa=0.865),Render had a general consistency with gold standard diagnosis results(Kappa=0.521).The sensitivity,specificity,accuracy and Yoden index of Omniview in diagnosing IUA were significantly higher than those of Render.ROC curve indicated that the area under curve(AUC)of Omniview diagnosis IUA is 0.927(95%CI:0.849~1.000),is significantly greater than the AUC for Render'diagnosis of IUA is 0.816(95%CI:0.728~0.935).The sensitivity,specificity,accuracy and Yoden index of Omniview in diagnosing mild,moderate and severe IUA were all higher than those of Render.Conclusions Compared with the traditional Render,Omniview technology has higher diagnostic value for IUA.
