光线性角化病皮肤镜与组织病理特征的相关性研究

摘　　要：目的 探讨光线性角化病皮肤镜与组织病理特征的相关性。方法 收集山西医科大学第一医院皮肤性病科门诊临床诊断为光线性角化病的37例患者,先后行皮肤镜和组织病理诊断、分级,以组织病理诊断结果为金标准,计算皮肤镜诊断光线性角化病的灵敏度、特异度等。探讨皮肤镜与组织病理分级诊断的一致性及皮肤镜特征与组织病理类型的相关性。结果皮肤镜诊断的灵敏度为85.19%、特异度为80.00%。皮肤镜与组织病理分级诊断具有中等一致性(P<0.05)。皮肤镜特征在各组织病理类型中的分布差异无统计学意义(P> 0.05)。结论 皮肤镜诊断光线性角化病效能较高,在分级诊断中皮肤镜也具有一定价值,尚不能认为光线性角化病皮肤镜特征与组织病理类型之间存在相关性。Objective To investigate the correlation between dermoscopic and histopathological features of actinic keratosis(AK).MethodsThe present study collected a total of 37 patients with clinically diagnosed AK at the frst hospital of shanxi medical university.All patients underwent both dermoscopy examinations and histopathological examinations,with histopathological results serving as the reference standard for diagnosing AK.Subsequently,the diagnostic performance of dermoscopy in identifying AK was further assessed,focusing on its sensitivity and specificity.In addition,The consistency of dermoscopic and histopathological grades and the correlation between dermoscopic features and histopathological types were further discussed.Results The dermoscopy exhibited a sensitivity of 85.19%and specificity of 80.00%for diagnosing AK.There was moderate consistency between dermoscopic and histopathological grades(P<0.05).The distribution of dermoscopic features did not show any significant variation across different histopathological types(P>0.05).Conclusions The dermoscopy exhibits high diagnostic effciency in AK and holds certain value for the grading diagnosis of AK..However,no significant correlation has been identified between the dermoscopic features and histopathological types in AK.
