脉冲染料激光治疗婴儿头皮血管瘤影响毛发生长的研究

摘　　要：目的探讨应用脉冲染料激光(PDL)治疗婴儿头皮血管瘤(IH)影响毛发生长的危险因素。方法通过回顾性研究38例接受PDL治疗并痊愈的头皮浅表血管瘤患儿,电话随访治疗结束以后至少2年的头发生长情况。结果随访时29例患儿头发生长基本恢复正常,6例患儿头发生长稀少,3例患儿头发完全缺失。瘤体面积≥1cm^(2),PDL治疗次数>7次,平均治疗能量≥10J/cm^(2)是影响毛发生长的主要风险因素(P<0.05)。结论对于头皮浅表血管瘤患儿,当瘤体面积≥1cm^(2)、瘤体厚度增加时,建议采用PDL与其他方法联合治疗,可以减少激光治疗次数与治疗能量,以避免激光治疗引起脱发的风险。Objective To investigate the risk factors associated with hair growth in the treatment of infantile scalp hemangioma using pulsed dye laser(PDL).Methods The study retrospectively examined 38 cases with superficial infantile scalp hemangioma who were treated with PDL and achieved recovery.The follow-up of hair growth was conducted for a minimum duration of 2 years post-treatment.Results Among the 38 cases,nearly normal hair growth was observed in 29 cases,while minimal hair growth was reported in 6 cases and complete hair loss occurred in 3 cases.The main risk factors influencing hair growth include tumor area≥1 cm^(2),treatment times of PDL>7,and average treatment energy≥10 J/cm^(2)(P<0.05).Conclusions For pediatric cases with superficial scalp hemangioma,it is recommended to use PDL in combination with other methods when the tumor area equals or exceeds 1 cm^(2)and the thickness of the tumor increases,thereby reducing the frequency and energy in laser therapy,thus minimizing the risk of laser-induced hair loss.
