传统再植、邻指指动脉为蒂的岛状皮瓣修复治疗掌侧皮肤动脉缺损伴断指再植的临床对比分析

摘　　要：目的:对比分析传统再植、邻指指动脉为蒂的岛状皮瓣修复治疗掌侧皮肤动脉缺损伴断指再植的临床疗效。方法:回顾性收集本院在2022年1月至2023年1月期间收治的80例掌侧皮肤动脉缺损伴断指再植的患者的临床资料。根据治疗方案的不同将患者分为对照组(行传统再植术,38例)和观察组(行邻指指动脉为蒂的岛状皮瓣修复治疗,42例)。6 m后随访患者回院复查,复查项目包括再植指体修复效果、再植指体的植体成活情况以及再植指体的功能情况。结果:观察组的再植指体修复优良率显著高于对照组(P<0.05)。观察组的植体成活率比对照组显著提高,血管危象、植体坏死率比对照组显著降低(P<0.05)。随访6 m后,观察组的指甲长度、指关节活动度均比对照组显著提高,两点分辨觉比对照组显著缩短(P<0.05)。结论:邻指指动脉为蒂的岛状皮瓣修复术能提高再植指体修复效果,改善再植指体的植体成活情况以及再植指体的功能。Objective:To compare and analyze the clinical effect of traditional replantation and island flap pedicled with adjacent finger artery in the treatment of palmar skin artery defect with severed finger replantation.Methods:The clinical data of 80 patients with palmar cutaneous artery defect with severed finger regrafting admitted to our hospital from January 2022 to January 2023 were retrospectively collected.Patients were divided into control group(conventional replantation,38 cases)and observation group(island flap pedicled with adjacent finger artery,42 cases)according to different treatment plans.After 6 months of treatment,the patients were invited back to the hospital for review,including the repair effect of the retransplanted finger,the survival of the retransplanted finger,and the function of the retransplanted finger.Results:The excellent rate of finger replantation repair in observation group was significantly higher than that in control group(P<0.05).The graft survival rate of observation group was significantly higher than that of control group,and the rate of vascular crisis and graft necrosis was significantly lower than that of control group(P<0.05).The nail length and the range of motion of the finger joint in the observation group were significantly higher than those in the control group,and the two-point resolution was significantly shorter than that in the control group(P<0.05).Conclusion:The island flap pedicled with adjacent finger artery can improve the repair effect of retransplanted finger,improve the survival of retransplanted finger and the function of retransplanted finger.
