老年T2DM患者应用精蛋白重组人胰岛素混合注射液、赖脯胰岛素混合注射液治疗的临床效果对比

摘　　要：目的:比较精蛋白重组人胰岛素(甘舒霖40R)、赖脯胰岛素(优泌乐25R)治疗老年2型糖尿病(Type2 diabetes mellitus,T2DM)患者临床效果。方法:选取我院2021年1月至2024年1月接受治疗的老年T2DM患者60例,随机信封法分为对照组(优泌乐25R,n=30)和观察组(甘舒霖40R,n=30)。治疗前、治疗2 m后采用生化分析仪测患者空腹血糖(Fasting plasma glucose,FPG)、餐后2 h血糖(2 hours plasma glucose,2hPG)、糖化血红蛋白(Glycosylated Hemoglobin Antibody,HbAlc)、空腹胰岛素(Fasting insulin,FINS)水平,计算胰岛素抵抗指数(Insulin resistance index,HOMA-IR),总胆固醇(Total cholesterol,TC)、甘油三酯(Triglyceride,TG)水平,治疗2 m后记录疗效及低血糖事件。结果:与治疗前相比,各治疗组的FPG、2 hPG、HbAlc、FINS、HOMA-IR、TC、TG水平均明显降低(P<0.05),其中观察组FPG、2hPG、HbAlc水平显著低于对照组(P<0.05),FINS、HOMA-IR、TC、TG水平略低于对照组,但差异不显著(P>0.05);观察组疗效略高于对照组(P>0.05),低血糖发生率略低于对照组(P>0.05)。结论:甘舒霖40R、优泌乐25R可有效治疗老年T2DM患者,但甘舒霖40R效果更显著。Objective:To compare the clinical effects between Mixed Protamine Human Insulin Injection(Gansulin 40R)and Mixed Protamine Zinc Recombinant Human Insulin Lispro Injection(25R)in the treatment of elderly patients with type 2 diabetes mellitus(T2DM).Methods:Sixty elderly patients with T2DM who received treatment in our hospital from January 2021 to January 2024 were selected as the study subjects.They were randomly divided into a control group(25R,n=30)and an observation group(Gansulin 40R,n=30)using the envelope method.The levels of fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2hPG),glycosylated hemoglobin(HbAlc),and fasting insulin(FINS)were measured using a biochemical analyzer before treatment and after 2 months of treatment.Insulin resistance index(HOMA-IR)was calculated,and the levels of total cholesterol(TC)and triglyceride(TG)were detected.Clinical efficacy and the incidence of hypoglycemia after 2 months of treatment were recorded.Results:After treatment,there were significant decreases in the levels of FPG,2hPG,HbAlc,FINS,HOMA-IR,TC,and TG in each group(P<0.05).The levels of FPG,2hPG,and HbAlc in the observation group were significantly lower than those in the control group(P<0.05).The levels of FINS,HOMA-IR,TC,and TG were slightly lower than those in the control group,but the differences were not statistically significant(P>0.05).The effective rate of the observation group was slightly higher than that of the control group(P>0.05).The incidence of hypoglycemia was slightly lower than that in the control group(P>0.05).Conclusion:Both Gansulin 40R and 25R are effective in the treatment of elderly patients with T2DM,but the former is more effective.
