腹腔镜联合十二指肠镜治疗胆总管结石的临床疗效及安全性分析

摘　　要：目的:分析腹腔镜联合十二指肠镜治疗胆总管结石的临床疗效及安全性。方法:回顾性收集本院自2019年5月-2023年1月收治的102例胆总管结石患者的临床资料。依据手术方式的不同,将患者分为对照组(采用传统开腹手术,45例)和试验组(采用LC+ES治疗,47例)。分析对比两组的手术相关指标、术后结石取净率、残余结石发生率及并发症发生率。结果:试验组的住院、手术、首次排气时间均比对照组显著缩短,术中失血量比对照组显著降低(P<0.05)。试验组、对照组结石一次取净率、残余结石发生率相比较,试验组结石一次取净率更高,残余结石发生率更低(P>0.05)。试验组、对照组并发症发生率相比较,试验组并发症发生率(2.12%)比对照组并发症发生率(24.44%)低(P<0.05)。结论:LC联合EST治疗胆总管结石,能提高疗效,减少创伤、促进恢复和预后,减少并发症。Objective:To analyze the clinical efficacy and safety of laparoscopy combined with duodenoscope in the treatment of choledocholithiasis.Methods:The clinical data of 102 patients with choledocholithiasis admitted to our hospital from May 2019 to January 2023 were retrospectively collected.According to different surgical methods,the patients were divided into control group(45 cases with traditional open surgery)and experimental group(47 cases with LC+ES treatment).The related indexes,postoperative stone removal rate,residual stone incidence and complication rate of the two groups were analyzed and compared.Results:The time of hospitalization,operation and first exhaust in experimental group was significantly shorter than that in control group,and the intraoperative blood loss was significantly reduced(P<0.05).The rate of stone removal and the incidence of residual stones in experimental group were higher and the incidence of residual stones in experimental group was lower(P>0.05).The complication rate of experimental group(2.12%)was lower than that of control group(24.44%)(P<0.05).Conclusion:LC combined with EST in the treatment of choledocholithiasis can improve the curative effect,reduce trauma,promote recovery and prognosis,and reduce complications.
