丧偶冲击增加了老年人的医疗服务利用吗?

摘　　要：本文基于2011-2018年中国健康与养老追踪调查数据,采用双重差分和事件研究法分析了丧偶对中国老年人医疗服务利用的影响,并进一步分析了丧偶影响老年人医疗服务利用的渠道。结果表明:丧偶使得老年人过去一年的住院概率提高6.2%和住院支出增加36.5%,最近一次住院晚数增加0.574晚和住院支出增加45.4%。从动态变化结果来看,医疗服务利用增加主要来自于丧偶后的第一期和第二期,丧偶后第三期并没有显著增加。渠道分析结果表明,丧偶后老年人医疗服务利用的增加主要是因为老年人健康状况恶化而非代际支持和居住安排的变化导致。最后,本文提出了改善丧偶老年人健康的政策建议。Based on the data of China Health and Retirement Longitudinal Study(CHARLS)from 2011 to 2018,this paper uses the methods of Difference-in-Difference and Event-Study to analyze the impact of widowhood(widow and widower)on medical utilization of the elderly in China.The results show that,widowhood increase the hospitalization probability and the expenditure on hospitalization in the past year by6.2%and 36.5%respectively,and increase the latest hospital stay and the latest expenditure on hospitalization by 0.574 nights and 45.4%respectively.The dynamic reviews show that these increases mainly come from the first and second phases of widowhood while no increase caused by the third phase of widowhood.Further analysis show that the increase of medical utilization of the widowed elderly is caused by the deterioration of their health status instead of the changes in the generational supports and the living arrangement.Some policy recommendations are put forth to promote the health of the widowed elderly too.
