基于数据挖掘的汪受传教授调治小儿乳蛾的处方用药规律

摘　　要：目的:通过数据挖掘探讨汪受传教授调治小儿乳蛾的处方用药规律,为临床治疗提供选方用药和思路借鉴。方法:收集汪受传教授临床诊治小儿乳蛾的病案,采用频数分析、关联规则和聚类分析等进行数据挖掘。结果:本研究共纳入161例乳蛾患儿,461张处方,涉及中药153味。高频药物包括蒲公英、芦根、桔梗、虎杖、桑白皮、金银花、黄芪等。按功效大致可归为清热药、化痰止咳平喘药、解表药、补虚药等19类。使用药物以寒性,甘、苦、辛味为主,归肺、胃、肝经居多,关联规则分析得出5组常用药对,系统聚类分析得出3个核心方。结论:汪受传教授认为小儿乳蛾病位在肺、胃、肝,病机为肺卫不固,热结咽喉,分急性发作期、发作间歇期和慢性迁延期三期论治。常用方剂为银翘散、五味消毒饮、玉屏风散、桂枝加龙骨牡蛎汤及泻白散,且常用药物的用量相对较小。Objective:To mine the medication rules of professor WANG Shouchuan in treating tonsillitis in children and provide references for the selection of prescriptions and ideas in clinical treatment.Methods:The clinical records of tonsillitis in children treated by professor WANG Shouchuan were collected.Data mining was carried out by frequency analysis,association rule analysis,and cluster analysis.Results:A total of 161 patients were included,and 461 effective prescriptions were collected,involving 153 Chinese medicines.High-frequency Chinese medicines included Taraxaci Herba,Phragmitis Rhizoma,Platycodonis Radix,Polygoni Cuspidati Rhizoma et Radix,Mori Cortex,Lonicerae Japonicae Flos,and Astragali Radix.The Chinese medicines were classified into 19 types according to the functions including heat clearing,phlegm resolving,cough stopping,and asthma relieving,exterior releasing,and tonifying.The Chinese medicines used were mainly cold,with sweet,bitter,and pungent tastes,and tropism to the lung,stomach,and liver meridians.Five commonly used Chinese medicine combinations were obtained by the association rule analysis,and three core prescriptions were revealed by the cluster analysis.Conclusion:Professor WANG Shouchuan held that the tonsillitis in pediatric mainly involved the lung,stomach,and liver,with the basic pathogenesis of insecurity of lung defense and heat accumulation in the throat.Staged treatment can be carried out regarding the acute attack,intermittment,and chronic persistent stages.The commonly used prescriptions were Yinqiao Powder,Wuwei Xiaodu Decoction,Yupingfeng Powder,Guizhi Longgu Muli Decoction,and Xiebai Powder,and the doses of the commonly used Chinese medicines were relatively small.
