纳布啡与舒芬太尼复合丙泊酚对宫腔镜患者血流动力学及疼痛、不良反应的影响

摘　　要：目的分析纳布啡与舒芬太尼分别复合丙泊酚在宫腔镜手术中的麻醉作用。方法纳入2021年6月至2023年6月于本院行宫腔镜手术的患者80例,以麻醉用药情况作为分组依据,对照组40例接受舒芬太尼复合丙泊酚方案,观察组40例接受纳布啡复合丙泊酚方案,对比不同阶段的收缩压、舒张压和心率、镇痛与镇静效果、炎症因子水平和不良反应发生率。结果观察组术中、手术结束时和麻醉苏醒时收缩压、舒张压和心率更低(P<0.05);较之于对照组,观察组苏醒后5 min和出手术室时Ramsay镇静评分(RSS)更高,术后6 h、术后12 h和术后24 h视觉模拟评分法(VAS)评分更低(P<0.05);观察组术后24 h指标炎症因子水平更低(P<0.05);两组不良反应发生率对比,观察组发生率更低(P<0.05)。结论纳布啡复合丙泊酚可稳定血压、心率指标,改善疼痛、镇静评分,降低不良反应发生率。Objective To analyze the anesthetic effects of naborphanine and sufentanil combined with propofol in hysteroscopic surgery.Methods Eighty confirmed patients were included in the study,all of whom underwent hysteroscopic surgery from June 2021 to June 2023 at our hospital.Anesthetic medication was used as the grouping basis.The control group(n=40)received a combination of sufentanil and propofol regimen,while the observation group(n=40)received a combination of nalbuphine and propofol regimen.The systolic blood pressure,diastolic blood pressure,heart rate,analgesic and sedative effects,inflammatory cytokine levels,and incidence of adverse reactions were compared at different stages.Results The comparison of systolic blood pressure,diastolic blood pressure,and heart rate between the two groups before anesthesia was consistent(P>0.05),while the observation group had lower systolic blood pressure,diastolic blood pressure,and heart rate during surgery,at the end of surgery,and at anesthesia awakening(P<0.05);Compared to the control group,the Ramsay sedation score was higher and the VAS score for pain was lower in the observation group(P<0.05);Comparison of immediate postoperative C-reactive protein,interleukin-6,and tumor necrosis factor-αFactor level,no difference(P>0.05),the observation group had lower levels of indicators 24 hours after surgery(P<0.05);There was a difference in the incidence of adverse reactions between the two groups(P<0.05).Conclusions The combination of nalbuphine and propofol can stabilize blood pressure and heart rate indicators,improve pain and sedation scores,and reduce the incidence of adverse reactions.
