硝苯地平联合硫酸镁治疗妊娠期高血压患者的疗效分析

摘　　要：目的分析硝苯地平联合硫酸镁治疗妊娠期高血压(PIH)患者的疗效。方法选取贵州省黔西南州人民医院产科2022年1月至2023年12月收治的100例PIH患者,根据不同疗法纳入对照组(50例)与观察组(50例),对照组予以硝苯地平治疗,观察组予以硝苯地平联合硫酸镁治疗,对比两组的血压水平(SBP、DBP)、尿肌酐与尿蛋白水平、心率(产妇、胎儿)、临床疗效、不良反应(恶心呕吐、头晕、皮肤瘙痒、消化道出血)、不良妊娠结局(宫颈裂伤、过度宫缩、急产、产后出血、新生儿窒息)。结果两组治疗后的SBP、DBP均降低,且观察组低于对照组,P<0.05;两组治疗后的尿肌酐与尿蛋白均降低,且观察组低于对照组,P<0.05;两组治疗后产妇、胎儿的心率均降低,且观察组低于对照组,P<0.05;观察组总有效率高于对照组,P<0.05;两组不良反应发生率相近,P>0.05;观察组不良妊娠结局发生率低于对照组,P<0.05。结论硝苯地平联合硫酸镁治疗PIH患者的疗效较好,可有效控制患者血压,改善尿肌酐与尿蛋白水平,降低母婴心率,改善妊娠结局,且不良反应少。Objective To analyze the efficacy of nifedipine combined with magnesium sulfate in the treatment of patients with gestational hypertension(PIH).Methods A total of 100 PIH patients admitted to the Obstetrics and Gynecology Department of Qiannan Prefecture People's Hospital in Guizhou Province from January 2022 to December 2023 were selected and included in control group(n=50)and observation group(n=50)according to different therapies.control group was treated with nifedipine,while observation group was treated with nifedipine combined with magnesium sulfate.The blood pressure levels(SBP,DBP),urinary creatinine and urinary protein levels,heart rate(maternal and fetal),clinical efficacy,adverse reactions(nausea and vomiting,dizziness,pruritus,gastrointestinal bleeding),adverse pregnancy outcomes(cervical laceration,excessive uterine contraction,emergency delivery,postpartum hemorrhage,neonatal asphyxia)of the two groups were compared.Results After treatment,SBP and DBP in both groups decreased,and observation group was lower than control group,with P<0.05;After treatment,the levels of urine creatinine and urine protein decreased in both groups,and observation group was lower than control group,with P<0.05;After treatment,the heart rates of both the mother and the fetus decreased in both groups,and observation group was lower than control group,with P<0.05;The total effective rate of observation group was higher than that of control group,with P<0.05;The incidence of adverse reactions in the two groups was similar,P>0.05;The incidence of adverse pregnancy outcomes in observation group was lower than that in control group,with P<0.05.Conclusions The combination of nifedipine and magnesium sulfate has a good therapeutic effect on PIH patients,which can effectively control blood pressure,improve urinary creatinine and protein levels,reduce maternal and infant heart rate,improve pregnancy outcomes,and have fewer adverse reactions.
