骨水泥联合型股骨近端防旋髓内钉固定治疗老年骨质疏松性股骨转子间骨折的临床效果

摘　　要：目的分析给予老年骨质疏松性股骨转子间骨折患者骨水泥联合型股骨近端防旋髓内钉(PFNA)固定治疗的效果。方法选取2020年11月至2023年11月93例老年骨质疏松性股骨转子间骨折患者为研究对象,入院后,按照随机数字表法分为对照组(n=46)、试验组(n=47)两组,对照组给予股骨近端防旋髓内钉固定术,试验组在其基础上增加骨水泥强化固定,对比治疗有效率、各项手术指标、髋关节功能、并发症发生率与内固定物失效率。结果试验组治疗总有效率以及Harris髋关节功能量表(HHS)评分较对照组相比更高(均P<0.05);试验组较对照组的下床行走锻炼时间、恢复正常行走时间、骨折愈合时间更短(均P<0.05);试验组手术并发症发生率与内固定物失效率均低于对照组(均P<0.05)。结论骨水泥联合型PFNA固定治疗能够对老年骨质疏松性股骨转子间骨折起到显著疗效,可加速患者肢体功能与髋关节功能的康复,减轻患者的疼痛感与不适感。Objective To analyze the effect of bone cement combined with proximal anti-rotation intramedullary nail(PFNA)fixation in elderly patients with osteoporotic intertrochanteric fracture of femur.Methods A total of 93 elderly patients with osteoporotic intertrochanteric fracture of femur from November 2020 to November 2023 were selected as the study subjects.After admission,they were divided into control group(n=46)and experimental group(n=47)according to random number table method.The control group received proximal femoral anti rotation intramedullary nail fixation surgery,while the experimental group received additional bone cement reinforcement fixation.The treatment efficacy,various surgical indicators,hip joint function,incidence of complications,and internal fixation failure rate were compared.Results The total effective rate of treatment and Harris Hip Scale(HHS)score were higher in the experimental group compared with the control group(both P<0.05);the time to get out of bed and walk,the time to return to normal walking,and the time to fracture healing were shorter in the experimental group compared with the control group(both P<0.05);and the rate of surgical complications and the rate of failure of internal fixation in the experimental group were lower than that of the control group(both P<0.05).Conclusions The combination of bone cement and PFNA fixation can have a significant therapeutic effect on elderly patients with osteoporotic intertrochanteric fractures,accelerate the recovery of limb and hip joint functions,and reduce pain and discomfort in patients.
