穴位贴敷联合手法按摩预防髋部骨折术后深静脉血栓临床观察

摘　　要：目的探究穴位贴敷联合手法按摩对髋部骨折术后深静脉血栓(DVT)的预防效果。方法选取2021年10月—2022年12月山东省单县中医医院收治的髋部骨折患者98例,按照随机数字表法分为对照组和观察组,各49例。对照组采取常规预防措施,观察组在对照组基础上加用穴位贴敷联合手法按摩,比较两组凝血功能,血流速度,肢体疼痛、肿胀程度及DVT发生率。结果干预后,观察组活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、凝血酶原时间(PT)长于对照组,纤维蛋白原(FIB)、D-二聚体(D-D)水平低于对照组(P<0.05);观察组下肢腘静脉血流速度、股静脉血流速度高于对照组,视觉模拟量表(VAS)评分低于对照组,下肢周径差小于对照组(P<0.05)。观察组DVT发生率为2.04%(1/49),低于对照组的16.33%(8/49)(P<0.05)。结论穴位贴敷联合手法按摩能够改善髋部骨折手术患者凝血功能与下肢血流速度,减轻下肢疼痛、肿胀,有效预防DVT发生。Objective To explore the preventive effect of acupoint application combined with manual massage on deep vein thrombosis(DVT)after hip fracture surgery.Methods The 98 patients with hip fractures admitted to Shan County Hospital of Traditional Chinese Medicine from October 2021 to December 2022 were selected and randomly divided into two groups,with 49 patients in each group.The control group received routine preventive measures,while the observation group received acupoint application combined with manual massage.The coagulation function,blood flow velocity,limb pain and swelling,and the incidence of DVT were compared between the two groups.Results After intervention,the activated partial thromboplastin time(APTT),thrombin time(TT)and prothrombin time(PT)in observation group were longer than those in control group,and the levels of fibrinogen(FIB)and D-dimer(D-D)in observation group were lower than those in control group(P<0.05).The blood flow velocity of popliteal vein and femoral vein in observation group was higher than that in control group,the score of visual analog scale(VAS)was lower than that in control group,and the difference of circumference diameter of lower limb was lower than that in control group(P<0.05).The incidence of DVT in the observation group was 2.04%(1/49),which was lower than that in the control group(16.33%,8/49)(P<0.05).Conclusion Acupoint application combined with manual massage can improve coagulation function and lower limb blood flow velocity in patients undergoing hip fracture surgery,reduce lower limb pain and swelling,and effectively prevent the occurrence of DVT.
