益气活血通络汤联合替罗非班治疗进展性卒中临床观察

摘　　要：目的探讨益气活血通络汤联合替罗非班治疗进展性卒中(SIP)的效果。方法选取2020年5月—2022年5月九江市中医医院收治的60例SIP患者作为研究对象,按随机数字表法分为两组,各30例。对照组予以盐酸替罗非班氯化钠注射液治疗,观察组加用益气活血通络中药治疗,两组均治疗14 d。比较两组临床疗效、神经功能、改良Rankin量表(MRS)评分、洼田饮水试验评分、中医证候积分、血小板功能及安全性。结果观察组总有效率93.33%(28/30)高于对照组的73.33%(22/30)(P<0.05)。治疗后,观察组美国国立卫生研究院卒中量表(NIHSS)评分、MRS评分、洼田饮水试验评分低于对照组,半身不遂、偏身麻木、气短乏力、舌歪语謇证候积分低于对照组,差异有统计学意义(P<0.05);治疗后,观察组血小板聚集率、血小板黏附率低于对照组,差异有统计学意义(P<0.05)。结论益气活血通络汤联合替罗非班治疗SIP可增强临床效果、加快患者神经功能恢复、纠正血小板功能异常、降低中医证候积分,且安全可靠。Objective To investigate the therapeutic effect of Yiqi Huoxue Tongluo decoction combined with tirofiban on stroke in progression(SIP).Methods The 60 patients with SIP admitted to Jiujiang Hospital of Traditional Chinese Medicine from May 2020 to May 2022 were randomly divided into two groups,with 30 patients in each group.The control group was treated with tirofiban,and the observation group was treated with Chinese medicine for supplementing qi,activating blood circulation and dredging collaterals for 14 days on the basis of the control group.The clinical efficacy,neurological function,modified Rankin Scale(MRS)score,Water Swallowing Test(WST)score,TCM syndrome score,platelet function and safety were compared between the two groups.Results The total effective rate of observation group was 93.33%(28/30),which was higher than that of control group(73.33%,22/30)(P<0.05).After treatment,the score of National Institutes of Health Stroke Scale(NIHSS),MRS Score and WST score in the observation group were lower than those in the control group,and the scores of hemiparalysis,heminumbness,shortness of breath,and tongue and speech were lower than those in the control group,and the difference was statistical significance(P<0.05).After treatment,the platelet aggregation rate and platelet adhesion rate in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Yiqi Huoxue Tongluo decoction combined with tirofiban can enhance the therapeutic effect of SIP,accelerate the recovery of patients’neural function,correct abnormal platelet function,and reduce the integral of TCM syndromes,which is safe and reliable.
