通脉消瘀汤足浴联合踝泵运动对下肢静脉曲张术后患者康复的影响

摘　　要：目的观察通脉消瘀汤足浴联合踝泵运动对下肢静脉曲张(LEVV)术后患者康复的影响。方法将86例LEVV术后患者按掷硬币法随机分为2组。治疗组43例在常规干预基础上加通脉消瘀汤足浴联合踝泵运动,对照组43例予常规干预。2组均干预30天。比较2组干预前和干预后15、30天疼痛视觉模拟评分(VAS)、下肢静脉曲张严重程度量表(RVCSS)评分变化;比较2组干预前和干预后15、30天血流动力学[静脉回流时间(VFT)、静脉容量(VC)、静脉内径(VD)]变化;比较2组干预前和干预后15、30天静脉功能不全的流行病学和经济研究-生活质量(VEINES-QOL)评分变化。结果2组干预15、30天疼痛VAS、RVCSS评分均较本组干预前降低(P<0.05),且有降低趋势(P<0.05);干预15、30天治疗组疼痛VAS、RVCSS评分均低于对照组干预同期(P<0.05)。2组干预15、30天VFT、VC、VD均较本组干预前降低(P<0.05),且呈降低趋势(P<0.05);干预15、30天治疗组VFT、VC、VD均低于对照组干预同期(P<0.05)。2组干预15、30天VEINES-QOL评分均较本组干预前升高(P<0.05),且呈升高趋势(P<0.05);干预15、30天治疗组VEINES-QOL评分均高于对照组干预同期(P<0.05)。结论通脉消瘀汤足浴联合踝泵运动可缓解LEVV术后患者疼痛程度,改善血流动力学水平,提高生活质量。Objective To observe the application of herbal foot bath therapy with Tongmai Xiaoyu Decoction plus ankle pump exercise in postoperative nursing in patients with lower extremities varicose veins(LEVV).Methods A total of 86 patients with LEVV were enrolled.The patients were randomly divided into control group(n=43)and experimental group(n=43)by a coin tossing method.All patients were managed by routine intervention,those in the experimental group additionally received herbal foot bath therapy with Tongmai Xiaoyu Decoction plus ankle pump exercise.A 30-day intervention was performed,aiming to compare Visual Analogue Scale(VAS)for pain,revised venous clinical severity score(RVCSS),hemodynamics(venous return time[VFT],venous volume[VC],venous diameter[VD]),Venous Insufficiency Epidemiology and Economic Study-Quality of Life(VEINES-QOL)before and after intervention between groups.Results After intervention,the 15-day and 30-day scores of VAS and RVCSS,and the 15-day and 30-day levels of VFT,VC and VD in the two groups were significantly decreased than those before intervention,and which showed a significant decreasing trend with time(P<0.05),and the decreases were more pronounced in the experimental group relative to the control group(P<0.05).The 15-day and 30-day scores of VEINES-QOL in the two groups were significantly increased than those before intervention,and which showed a significant increasing trend with time(P<0.05),and the increases were more common in the experimental group compared to the control group(P<0.05).Conclusion For patients with LEVV,herbal foot bath therapy with Tongmai Xiaoyu Decoction plus ankle pump exercise can effectively relieve postoperative pain degree of patients,the level of hemodynamics and quality of life.
