摘 要:目的评价3D打印辅助术前设计个体化钢板在髋臼后壁骨折手术治疗中的临床应用效果。方法回顾性分析2016年2月—2022年12月于张家港市第一人民医院骨科住院治疗的61例髋臼后壁骨折患者的临床资料,按手术方法分为3D组和传统组。3D组(n=31)根据患者3D打印骨折模型设计个体化钢板,传统组(n=30)采用传统手术方法。比较两组患者的骨折解剖复位结果、髋关节功能恢复评分、手术时间、出血量及术后并发症发生情况。结果3D组的骨折复位优良率为96.77%,显著优于传统组的83.33%,差异有统计学意义(χ^(2)=6.207,P<0.05)。3D组术后3、6、12个月复查髋关节功能优良率均在93.00%以上,均显著高于传统组,差异有统计学意义(P均<0.05)。3D组术中出血量少于传统组,差异有统计学意义(t=10.557,P<0.05)。3D组的术后并发症总发生率为3.23%,显著低于传统组的20.00%,差异有统计学意义(χ^(2)=5.613,P<0.05)。结论3D打印辅助术前设计可提高髋臼后壁骨折患者的骨折复位质量和髋关节功能恢复,增强手术过程的精准性和安全性。Objective To evaluate the clinical application effect of 3D printing-assisted preoperative design of individualized steel plates in the surgical treatment of posterior acetabular wall fractures.Methods The clinical data of 61 cases of posterior acetabular wall fractures hospitalized in the Department of Orthopedics of Zhangjiagang First People's Hospital from February 2016 to December 2022 were retrospectively selected and were divided into 3D group and traditional group according to surgical methods.Among them,the 3D group(n=31)designed individualized plates according to the patients'3D-printed fracture models,and the traditional group(n=30)used traditional surgical methods.The fracture anatomical reduction results,hip function recovery scores,operation time,bleeding and postoperative complications occurred in the two groups were compared.Results The excellent rate of fracture reduction in the 3D group was 96.77%,which was significantly better than 83.33%in the traditional group,the difference was statistically significant(χ^(2)=6.207,P<0.05).The excellent and good rate of hip function in the 3D group was more than 93.00%at 3,6 and 12 months after operation,which was significantly higher than that of the traditional group,the differences were statistically significant(all P<0.05).The intraoperative bleeding volume of the 3D group were less than those of the traditional group,the difference was statistically significant(t=10.557,P<0.05).And the incidence of postoperative complications in the 3D group was 3.23%,which was significantly lower than that of 20.00%in the traditional group,the difference was statistically significant(χ^(2)=5.613,P<0.05).Conclusion 3D printing-assisted preoperative design can improve the quality of fracture reduction and recovery of hip joint function in patients with posterior acetabular wall fractures,and enhance the precision and safety of the surgical process.
