PFNA、PFALP治疗老年人股骨转子间骨折的临床疗效及对并发症发生率的影响分析

摘　　要：目的比较防旋股骨近端髓内钉(proximal femoral nail antirotation,PFNA)、股骨近端解剖锁定钢板(proximal femoral anatomic locking plate,PFALP)治疗老年人股骨转子间骨折的临床疗效及对并发症发生率的影响。方法回顾性选取2020年2月—2023年2月京山市人民医院收治的180例老年股骨转子间骨折患者的临床资料,以治疗方式差异分为研究组(行PFNA治疗)和对照组(行PFALP治疗),各90例。对比两组骨折恢复优良率、临床指标、疼痛评分及并发症情况。结果研究组的骨折恢复优良率为88.89%,明显高于对照组的77.78%,差异有统计学意义(χ^(2)=4.000,P<0.05)。研究组的临床指标、视觉模拟评分法评分及并发症总发生率明显低于对照组,差异有统计学意义(P均<0.05)。结论相比PFALP,PFNA治疗老年股骨转子间骨折的效果更好,安全性更高,可促进患者早日康复。Objective To analyze the clinical effect of proximal femoral nail antirotation(PFNA)and proximal femoral anatomic locking plate(PFALP)in the treatment of intertrochanteric femoral fracture in the elderly and their influence on the incidence of complications.Methods The clinical data of one hundred and eighty elderly patients with intertrochanteric fracture of femur treated in Jingshan People's Hospital from February 2020 to February 2023 were retrospectively selected,and they were divided into study group(treated with PFNA)and control group(treated with PFALP)according to the difference in treatment methods,with ninety cases each.The excellent and good rate of fracture recovery,clinical indexes,pain score and complications were compared between the two groups.Results The fracture recovery rate of the study group was 88.89%,significantly higher than that of the control group(77.78%),and the difference was statistically significant(χ^(2)=4.000,P<0.05).The clinical indexes,scores of Visual Analogue Scale and total complication rate of the study group were significantly lower than those of the control group,and the differences were statistically significant(all P<0.05).Con⁃clusion Compared with PFALP,PFNA has better effect and higher safety in the treatment of elderly intertrochanteric fractures,and can promote the early recovery of patients.
