摘 要:目的通过与传统NPTR检测的对比,探讨实时睡眠监测联合夜间阴茎勃起检测(Nocturnal penile tumescence and rigidity,NPTR)在男性勃起功能障碍(Erectile dysfunction,ED)分型诊断中的应用。方法选取2023年1月-2023年7月合肥市第二人民医院接诊的64例男性ED患者作为研究对象。所有患者均进行至少两夜的NPTR检测并佩戴苹果智能穿戴Apple Watch Series 8,收集患者的勃起和睡眠数据并分析,评估患者的睡眠及勃起功能情况,确定心理性ED患者能达到有效勃起的有效睡眠临界值。对于前两夜未达到有效睡眠的患者,予以改善睡眠再次进行监测。采用实时睡眠监测联合NPTR(多夜)及单独NPTR(两夜)两种不同的方式对患者的ED分型进行诊断,并比较其诊断效能。对心理性ED患者的相关数据进行分析。结果对心理性ED患者的前后两夜监测数据进行分析发现,快速动眼睡眠与有效勃起事件最具相关性。仅应用NPTR检测,64例男性ED患者中33例(51.6%)被诊断为心理性ED,31例(48.4%)诊断为器质性ED;联合Apple Watch Series 8实时睡眠监测后,39例(60.9%)被诊断为心理性ED,25例(39.1%)诊断为器质性ED,其阴性预测值(Negative predictive value,NPV)为100.0%(25/25)。实时睡眠监测联合NPTR诊断ED分型的诊断效能远高于传统NPTR检测。进一步分析发现,快速动眼睡眠(Rapid eye movement,REM)对有效勃起事件有较高的预测价值(AUC:0.895,P<0.001,最佳界值为0.609,此时敏感性为71.1%,特异性为89.7%,Cut-off值为71.0)。结论相较于传统两夜的NPTR监测,NPTR联合睡眠实时监测具有更高的诊断价值。Objective To explore the application of Apple Watch real-time sleep monitoring combined with NPTR in the typing diagnosis of male Erectiledysfunction(ED)by comparing with traditional Nocturnal penile erection and rigidity(NPTR).Methods 64 male ED patients from January 2023 to July 2023 in Hefei Second People's Hospital were selected as the study objects.All patients underwent NPTR detection for at least two nights and wore Apple Watch Series 8.Erection and sleep data of patients were collected and analyzed to evaluate the patients'sleep and erectile function and determine the effective sleep threshold for effective erection in patients with psychological ED.Patients who did not achieve effective sleep in the previous two nights were monitored again to improve their sleep.Two different methods of Apple Watch real-time sleep monitoring combined with NPTR(multi-night)and NPTR(two-night)were used to diagnose ED types of patients,and their diagnostic efficiency was compared.The relevant data of patients with psychological ED were analyzed.Results The analysis of the two night monitoring data of patients with psychological ED showed that REM was most correlated with effective erectile events.Using NPTR alone,33(51.6%)of 64 male ED patients were diagnosed with psychological ED and 31(48.4%)with organic ED.After combined with Apple Watch Series 8 real-time sleep monitoring,39 patients(60.9%)were diagnosed with psychological ED,25 patients(39.1%)were diagnosed with organic ED,its negative predictive value(NPV)is 100.0%(25/25).The diagnostic efficiency of Apple Watch real-time sleep monitoring combined with NPTR for ED classification is much higher than that of traditional NPTR detection.Further analysis showed that rapid eye movement(REM)sleep duration had a high predictive value for effective erectile events(AUC:0.895,P<0.001,optimal Cut-off value of 0.609,sensitivity of 71.1%,specificity of 89.7%,cut-off value of 71.0).Conclusion Compared with the traditional two-night NPTR monitoring,NPTR combined with Apple Watch real
