摘　　要：目的探讨经皮椎间孔镜下髓核摘除术与经皮椎间孔镜下椎板开窗髓核摘除术用于治疗腰椎间盘突出症的效果。方法选取2020年1月-2023年6月在银川市中医医院采用经皮椎间孔镜下髓核摘除术治疗的40例腰椎间盘突出症患者作为观察组,同期选取在该院采用经皮椎间孔镜下椎板开窗髓核摘除术进行治疗的40例腰椎间盘突出症患者作为对照组;回顾性分析两组临床资料。结果两组治疗总有效率、不良反应发生率比较,差异不显著(P>0.05)。观察组手术时间、术中出血量、住院时间显著少于对照组(P<0.05)。两组术前VAS、ODI、SF-36比较,差异不显著(P>0.05),与术前相比,术后各时间点VAS、ODI、SF-36评分均显著改善(P<0.05),且术后观察组VAS、SF-36评分显著优于对照组(P<0.05),两组术后各时间点ODI比较,差异不显著(P>0.05)。结论两种术式均具有较好的治疗效果,经皮椎间孔镜下髓核摘除术手术指标更优、疼痛缓解情况更好且患者生活质量更高,值得推广。Objective To evaluate the efficacy of Percutaneous Transforaminal Interlaminar Nucleus Pulposus Extraction(PTINPE)and Percutaneous Transpedicular Discectomy with Microendoscopic Fenestration(PTDMF)in the treatment of lumbar disc herniation(LDH).Methods From January 2020 to June 2023,40 patients with LDH treated with PTINPE were selected as observation group,at the same time,40 patients with LDH treated with PTDMF were selected as control group.The clinical data of the two groups were analyzed retrospectively.Results There was no significant difference in the total effective rate and adverse reaction rate between the two groups(P>0.05).The operation time,blood loss and hospital stay in the observation group were significantly shorter than those in the control group(P<0.05).There was no significant difference in VAS,ODI and SF-36 between the two groups before operation(P>0.05).After treatment,VAS,ODI and SF-36 scores were significantly improved(P<0.05),after treatment,the VAS and SF-36 scores in the observation group were significantly better than those in the control group(P<0.05).There was no significant difference in ODI between the two groups(P<0.05).Conclusion Both of the two methods have good therapeutic effect,and the Operation Index of percutaneous discectomy is better,the pain relief is better and the patients'life quality is higher,so it is worth popularizing.
