层级管理模式下责任制整体护理在输尿管结石手术患者围手术期的应用研究

摘　　要：目的研究并探讨层级管理模式下责任制整体护理在输尿管结石手术患者围手术期的应用价值。方法研究对象为2022年8月-2023年8月就诊于湖南省财贸医院泌尿外科接受钬激光碎石治疗手术的输尿管结石患者116例,将其进行随机分组,分为对照组和观察组,每组各有58例患者。对照组采取常规护理方法,观察组采取层级管理模式下责任制整体护理方法。比较两组患者在护理前和护理后24小时内超氧化物歧化酶(SOD)、丙二醛(MDA)、皮质醇(Cor)水平的变化,并统计、比较两组患者的并发症总发生率。结果在护理后24小时进行对比时,发现观察组的SOD水平较护理前显著降低,而MDA和Cor水平则显著增加。此外,在护理后24小时的观察组中发现,其MDA和Cor水平均低于对照组,而SOD水平则高于对照组(P<0.05)。对照组的首次进食时间、首次拔除尿管时间以及首次下床活动时间均明显长于观察组,且对照组的并发症发生率明显高于观察组,差异有统计学意义(P<0.05)。结论在输尿管结石手术患者围手术期的护理中,采用层级管理模式和责任制整体护理能够提高护理质量和安全性,减少手术并发症的发生,促进患者的术后恢复。Objective To study and explore the value of holistic care in perioperative patients undergoing ureteral stones.Methods The study subjects were 116 patients with ureteric stones who visited the department of urology from August 2022 to August 2023,and they were randomized.This study was divided into control group and observation group,with 58 patients in each group.The control group adopted the routine nursing method,and the observation group adopted the responsibility system under the hierarchical management mode.Changes in superoxide dismutase(SOD),malondialdehyde(MDA),and cortisol(Cor)levels before and within 24 hours of care,and the total complication rates were compared.Results When compared at 24 hours after care,SOD levels were significantly lower compared to before care,while MDA and Cor levels increased significantly.In addition,the MDA and Cor levels in the observation group were lower than those in the control group,while the SOD level was higher than those in the control group(P<0.05).In the control group,the time of first food intake,first catheter removal time and the first time getting out of bed activity time were significantly longer than that of the observation group,and the complication rate in the control group was significantly higher than that of the observation group,with statistically significant differences(P<0.05).Conclusion In the perioperative care of patients with ureteral stone surgery,the adoption of hierarchical management mode and responsibility system can improve the quality and safety of care,reduce the occurrence of surgical complications,and promote the postoperative recovery of patients.
