通天口服液联合倍他司汀治疗前庭性偏头痛的疗效观察

摘　　要：目的探讨通天口服液联合盐酸倍他司汀片治疗前庭性偏头痛的临床疗效。方法选取2020年1月—2022年12月上海市宝山区仁和医院治疗110例前庭性偏头痛患者,按照随机数字表法将患者分对照组(55例)和治疗组(55例)。对照组患者口服盐酸倍他司汀片,2片/次,2次/d。在此基础上,治疗组患者口服通天口服液,第1天于即刻、服药1、2、4h时口服通天口服液10mL,以后每6h口服10mL,之后10mL/次,3次/d。两组患者均连续治疗2个月。比较两组的临床疗效、椎基底动脉血流速度、头痛程度、眩晕程度、病症发作次数、睡眠质量。结果治疗组的明显改善率为83.64%,明显高于对照组的明显改善率65.45%(P<0.05)。治疗后,两组左侧大脑椎动脉(LVA)、右侧大脑椎动脉(RVA)、基底动脉(BA)平均血流速度、视觉模拟评分法(VAS)、眩晕障碍量表(DHI)评分、病症每月发作次数、匹兹堡睡眠质量指数量表(PSQI)评分均显著降低(P<0.05),且治疗组的各指标比对照组更低(P<0.05)。结论通天口服液联合盐酸倍他司汀片治疗前庭性偏头痛疗效较好,可改善椎基底动脉狭窄,有效减轻患者头痛、眩晕程度,对降低病症发作频率,改善患者睡眠质量有积极影响。Objective To explore the clinical efficacy of Tongtian Oral Solution combined with Betahistine Hydrochloride Tablets in treatment of vestibular migraine.Methods Patients(110 cases)with vestibular migraine in Shanghai Baoshan District Renhe Hospital from January 2020 to December 2022 were divided into control and treatment groups according to the random number table metho,and each group had 55 cases.Patients in the control group were po administered with Betahistine Hydrochloride Tablets,2 tablets/time,twice daily.Patients in the treatment group were po administered with Tongtian Oral Solution on the basis of the control group,Tongtian Oral Solution was taken orally 10 mL at 0 h,1 h,2 h,and 4 h on the first day,then 10 mL every 6 h,then 10 mL/time,three times daily.Patients in two groups were treated for 2 months.The clinical efficacies,the blood flow velocity of vertebrobasilar artery,the degree of headache,the degree of vertigo,the number of monthly episodes of symptoms,and sleep quality in two groups were compared.Results The improvement rate of the treatment group was 83.64%,which was significantly higher than 65.45%of the control group(P<0.05).After treatment,the average blood flow velocity of LVA,RVA,and BA,the VAS scores and DHI scores,the monthly frequency of symptoms and PSQI score of two groups were significantly decreased(P<0.05),and the indexes of the treatment group were lower than those of the control group(P<0.05).Conclusion Tongtian Oral Liquid combined with Betahistine Hydrochloride Tablets has a good effect on the treatment of vestibular migraine,which can improve vertebrobasilar artery stenosis,effectively reduce the degree of headache and vertigo,reduce the frequency of symptoms and improve the sleep quality of patients.
