王氏保赤丸联合酪酸梭菌二联活菌散治疗婴幼儿牛奶蛋白过敏性腹泻的临床研究

摘　　要：目的探讨王氏保赤丸联合酪酸梭菌二联活菌散治疗婴幼儿牛奶蛋白过敏性腹泻的临床疗效。方法回顾性选取2021年9月—2023年10月郑州大学附属儿童医院收治的婴幼儿牛奶蛋白过敏性腹泻患儿112例,根据治疗方式的不同分为对照组(56例)和治疗组(56例)。对照组口服酪酸梭菌二联活菌散,1袋/次,2次/d。在对照组的基础上,治疗组王氏保赤丸,6丸/次,1次/d。两组连服药7d。观察两组患者临床疗效,比较治疗前后两组患者缓解症状时间,嗜酸性粒细胞计数(EOS)、免疫球蛋白E(IgE)、粪便中嗜酸性粒细胞计数,及血清肿瘤坏死因子-α(TNF-α)、干扰素-γ(INF-γ)、转化生长因子-β(TGF-β)、白细胞介素-6(IL-6)水平。结果治疗后,治疗组临床总有效率为94.64%,明显高于对照组(83.93%,P<0.05)。治疗后,治疗组症状缓解时间均明显早于对照组(P<0.05)。治疗后,两组患者EOS、IgE、粪便EOS计数均明显低于治疗前(P<0.05),且治疗组均明显低于对照组(P<0.05)。治疗后,两组患者血清TGF-β、TNF-α、IL-6水平明显低于治疗前,而INF-γ水平明显高于治疗前(P<0.05),且治疗组血清学水平明显好于对照组(P<0.05)。结论酪酸梭菌二联活菌散与王氏保赤丸协同治疗,可显著缓解腹泻症状,明显提升患儿免疫能力,减弱炎性反应状态。Objective To explore the clinical effect of Wang's Baochi Pills combined with Combined Clostridium Butyricum and Bifidobacterium Powders in treatment of infantile milk protein allergic diarrhea.Methods A total of 112 infants with milk protein allergic diarrhea treated in Children’s Hospital Affiliated to Zhengzhou University from September 2021 to October 2023 were selected retrospectively,and they were divided into control group(56 cases)and treatment group(56 cases)according to the different treatment methods.Patients in the control group were po administered with Combined Clostridium Butyricum and Bifidobacterium Powders,1 bag/time,twice daily.Patients in the treatment group were po administered with Wang’s Baochi Pills on the basis of the control group,6 pills/time,once daily.Patients in two groups were treated for 7 d.After treatment,the clinical evaluations were evaluated,the symptom relief time,IgE,EOS,and fecal EOS,and the levels of serum TGF-β,TNF-α,IL-6 and INF-γin two groups before and after treatment were compared.Results After treatment,the clinical effective rate of the treatment group was 94.64%,which was significantly higher than that of the control group(83.93%,P<0.05).After treatment,the time of symptom relief in the treatment group was significantly earlier than that in the control group(P<0.05).After treatment,the counts of EOS,IgE and fecal EOS in two groups were significantly lower than those before treatment,and which in the treatment group were significantly lower than those in the control group(P<0.05).After treatment,the serum levels of TGF-β,TNF-αand IL-6 in two groups were significantly lower than those before treatment,but the level of INF-γwas significantly increased(P<0.05),and the level of serology in the treatment group was significantly better than that in the control group(P<0.05).Conclusion The combined treatment of Combined Clostridium Butyricum and Bifidobacterium Powders and Wangshi Baochi Pills can significantly relieve the symptoms of diarrhea,significantly impro
