基于三部三焦理论探索构建痹证的针灸辨治体系

摘　　要：《黄帝内经》及历代医家对痹证的疾病认识和针灸治疗有大量的论述,但较为零散和复杂,故其辨治体系一直尚未完善。文章根据痹证的发病规律及中医诊治理念,基于三部三焦理论,对五体痹和五脏痹进行“两体相合,且应两脏”的整合,探索构建了痹证的三部痹(皮脉痹、筋肉痹、筋骨痹)和三焦痹(心肺痹、肝脾痹、肝肾痹)辨证体系,并系统梳理五刺、九刺、十二刺等刺法,建立了包含五体刺法和五脏刺法的针灸辨治体系,以期为中医针灸临床治痹提供更为清晰的指导框架。Huangdi Neijing and the discussions by successive medical scholars on the understanding and acupuncture treatment of paralysis have a wealth of content,but it is rather fragmented and complex.Therefore,its diagnostic and treatment system has not been perfected.Based on the pathogenesis patterns of paralysis and the principles of traditional Chinese medicine diagnosis and treatment,and guided by the three parts and the three Jiao theories,this paper integrates five-body paralysis and five-organ paralysis into a system of‘two bodies in harmony,each corresponding to two organs'.It explores and constructs the three parts paralysis(dermal-meridian paralysis,tendon-muscle paralysis,tendon-skeletal paralysis)and three Jiao paralysis(cardiopulmonary,liver-spleen,liver-kidney)diagnostic system for paralysis.The paper systematically examines the five-prick methods,nine-prick methods,twelve-prick methods,establishing a acupuncture treatment system that includes both five-body and five-organ needling techniques.The aim is to provide a clearer guiding framework for the clinical acupuncture treatment of paralysis in traditional Chinese medicine.
