血清钙磷乘积对老年骨质疏松性椎体骨折的预测价值探讨

摘　　要：目的 分析血清钙磷乘积对老年骨质疏松性椎体压缩骨折(OVCF)的预测价值。方法 回顾性分析2018年7月至2022年6月收治的因老年OVCF行椎体成形术患者100例为观察组,选取同期同数量年龄、性别相匹配的因脊柱退变性疾病行手术治疗的患者为对照组,比较两组入院时的一般资料及实验室指标,通过Logistic回归分析老年OVCF发生的相关影响因素,绘制ROC曲线,比较其评估老年患者OVCF风险发生的效能,并与亚洲人骨质疏松症自我评估工具(OSTA)进行对比分析,进一步探讨血清钙磷乘积对老年OVCF的预测价值。结果 观察组白蛋白、血清钙、磷、钙磷乘积、校正血钙、校正钙磷乘积、OSTA指数均低于对照组(P<0.05)。Logistic回归分析表明,血清钙、磷、钙磷乘积、校正血钙、校正钙磷乘积及OSTA指数升高是老年OVCF的保护因素。ROC曲线结果显示,血清钙磷乘积、校正钙磷乘积及OSTA对老年OVCF诊断的AUC值分别为0.810、0.772、0.659,且血清钙磷乘积及校正钙磷乘积预测老年OVCF的效能优于OSTA指数。结论 血清钙磷乘积及校正钙磷乘积对老年OVCF的发生具有预测价值。Objective To investigate the predictive value of the serum calcium-phosphorus product for osteoporotic vertebral compression fractures in elderly individuals.Methods A retrospective analysis was conducted on 100 patients admitted for vertebroplasty due to OVCF in elderly individuals between July 2018 and June 2022,constituting the observation group.An equivalent number of age-and gender-matched patients undergoing surgical treatment for spinal degenerative diseases during the same period were selected as the control group.General demographic and laboratory data at the time of admission were compared between the two groups.Logistic regression was employed to analyze factors associated with the occurrence of OVCF in the elderly.Receiver Operating Characteristic curves were plotted to evaluate the effectiveness of these factors in assessing the risk of OVCF in elderly patients,and the results were compared to those of the Osteoporosis Self-Assessment Tool for Asians,a tool designed to assess OVCF risk in elderly patients of Asian descent.Results The observation group exhibited statistically significant lower levels of albumin,serum calcium,phosphorus,calcium-phosphorus product,corrected blood calcium,corrected calcium-phosphorus product,and OSTA index compared to the control group(P<0.05).Logistic regression analysis indicated that higher serum calcium,phosphorus,calcium-phosphorus product,corrected blood calcium,corrected calcium-phosphorus product,and OSTA index were protective factors against OVCF in the elderly.ROC curve results demonstrated that the Area Under the Curve values for serum calcium-phosphorus product,corrected calcium-phosphorus product,and OSTA in diagnosing OVCF in the elderly were 0.810,0.772,and 0.659,respectively.Notably,the predictive capabilities of the serum calcium-phosphorus product and corrected calcium-phosphorus product were superior to those of the OSTA index.Conclusion Serum calcium-phosphorus product and corrected calcium-phosphorus product hold predictive value for the occurrence o
