促凝方治疗毒蛇咬伤患者凝血功能异常的临床观察

摘　　要：目的观察促凝方在毒蛇咬伤所致凝血功能异常中的治疗效果。方法选取116例毒蛇咬伤患者作为研究对象,采用随机数字表法分为两组各58例。两组入院后均给予常规治疗,联合组另予促凝方治疗。比较两组治疗前,治疗第1、3、6日肿胀程度、凝血功能指标、血常规指标变化以及住院时间。结果常规组中有1例脱落,联合组中有4例脱落;治疗第3、6日两组肿胀程度均有所改善,且联合组治疗第3、6日肿胀程度等级分布优于常规组(P<0.05);联合组治疗第1日的凝血酶原时间以及治疗第3、6日的凝血酶原时间、活化部分凝血活酶时间、凝血酶时间均短于常规组,治疗第6日的纤维蛋白原水平高于常规组(P<0.05);联合组治疗第3日的红细胞计数、血红蛋白以及治疗第6日的血小板计数均高于常规组(P<0.05);联合组住院时间短于常规组(P<0.05)。结论毒蛇咬伤所致凝血功能异常患者在常规治疗基础上联合促凝方治疗能够促进肿胀缓解,改善凝血功能,缩短住院时间。Objective:To explore the therapeutic effect of traditional Chinese medicine Promoting Coagulation Formula on coagulation dysfunction caused by venomous snake bites.Methods:A total of 116 patients with venomous snake bites admitted were selected as the study subjects,and they were randomized into a conventional group(58 cases)and a combined group(58 cases)using a random number table method.After admission,both groups were given routine treatment,while the combined group was given Promoting Coagulation Formula.The swelling degree,coagulation function indicators,blood routine indicators,and hospitalization time between the two groups before treatment,Ist day of treatment,3rd day of treatment,6th day of treatment were compared.Results:There was 1 case of drop-out in the conventional group and 4 cases of drop-out in the combined group.The degree of swelling in both groups improved on the 3rd and 6th day of treatment,and the distribution of swelling levels in the combined group were better than those in the conventional group on the 3rd and 6th day of treatment(P<0.05).The combined group had shorter prothrombin time on the 1st day of treatment,as well as shorter prothrombin time,activated partial thromboplastin time,and thrombin time on the 3rd and 6th days of treatment compared with the conventional group.The fibrinogen level on the 6th day of treatment was higher than that of the conventional group(P<0.05).The red blood cell count,hemoglobin on the 3rd day of treatment,and platelet count on the 6th day of treatment in the combined group were higher than those in the conventional group(P<0.05).The hospitalization time in the combined group was shorter than that in the conventional group(P<0.05)J.Conclusion:On the basis of routine treatment for patients with abnormal coagulation function caused by venomous snake bites,the combination with Promoting Coagulation Formula can promote swelling relief,improve coagulation function,and shorten hospitalization time.
