摘　　要：目的探讨双嵌片颈椎融合器治疗连续双节段脊髓型颈椎病的临床疗效。方法采用双嵌片颈椎融合器治疗32例连续双节段脊髓型颈椎病患者。记录手术时间、术中出血量、术后并发症情况,采用JOA评分系统评定脊髓神经功能恢复情况,测量并计算椎间隙高度百分比、Cobb角。结果患者均获得12个月随访。手术时间60~115 min,术中出血量20~150 ml。术后无切口感染、吞咽困难、声音嘶哑、气管食管瘘等并发症发生。术后X线片显示:椎间隙高度基本恢复正常,融合器在位,未出现明显移位及下沉等情况。JOA评分、椎间隙高度百分比、Cobb角:术后2周、6个月、12个月均较术前改善(P<0.05)。术后12个月采用JOA评分系统评定脊髓神经功能恢复情况:优12例,良15例,可5例,优良率27/32。结论采用双嵌片颈椎融合器治疗连续双节段脊髓型颈椎病并发症少,椎体隙高度、脊髓神经功能、颈椎曲度恢复好。Objective To investigate the clinical effect of double-plate self-locking interbody fusion cage(ROI-C)for treatment of continuous double-level cervical spondylotic myelopathy.Methods The 32 cases of continuous double-level cervical spondylotic myelopathy were treated with ROI-C.The operative time,intraoperative blood loss and postoperative complications were recorded.The recovery of spinal nerve function was evaluated by JOA scoring system.The percentage of vertebral space height and Cobb angle were measured and calculated.Results All patients were followed up for 12 months.The operative time was 60~115 min and the intraoperative blood loss was 20~150 ml.There were no postoperative complications such as incision infection,dysphagia,hoarseness and tracheoesophageal fistula.Postoperative radiographs showed that the height of the intervertebral space was basically returned to normal,the fusion cage was in position,and no obvious displacement or subsidence occurred.JOA score,percentage of intervertebral height and Cobb angle were all improved at 2 weeks,6,12 months after surgery than the preoperation(P<0.05).Twelve months after surgery,the recovery of spinal nerve function was evaluated by JOA scoring system:12 cases were excellent,15 cases were good,5 cases were fair,and the rate of excellent and good was 27/32.Conclusions There are fewer complications and better recovery of vertebral space height,spinal nerve function and cervical curvature in the treatment of continuous double-level cervical spondylotic myelopathy with ROI-C.
