加味苓桂术甘汤治疗糖尿病前期临床观察

摘　　要：目的:观察加味苓桂术甘汤治疗糖尿病前期(脾虚痰湿证)的临床疗效及对患者血糖、血脂变化的影响。方法:将60例符合临床观察条件的患者随机分为治疗组、对照组两组,两组30例。两组患者均同样予以糖尿病饮食、适当运动等基础治疗措施,对照组患者给予口服二甲双胍缓释片治疗,治疗组患者在对照组基础上服用加味苓桂术甘汤治疗。两组患者在性别、年龄、病程、体质量指数、实验室检验指标、中医证候等方面差异均无统计学意义(P> 0.05),临床观察疗程均为28 d。结果:治疗后治疗组患者总有效率为93.33%(28/30),对照组患者总有效率为83.33%(25/30),总疗效及改善证候等的差异具有统计学意义(P <0.05)。与对照组患者相比较,治疗组患者治疗后空腹血糖(Glucose,GLU)、餐后2 h血糖(2-hour Plasma Glucose,2 h PG)、总胆固醇(Total Cholesterol,TC)、低密度脂蛋白胆固醇(Low-density Lipoprotein Cholesterol,LDL-C)均明显下降,差异具有统计学意义(P <0.05)。结论:糖尿前期患者存在程度不同的脾阳虚痰湿阻遏等状态,单纯西药治疗效果较差,联合中医药辨证治疗可以显著提高临床疗效及改善患者症状,调节血糖、血脂等。加味苓桂术甘汤为中医经方加减,治疗糖尿病前期患者(脾虚痰湿证)的临床疗效优于单纯西药,并且能够明显改善患者临床症状,降低血糖、血脂等,值得在临床上进一步推广应用。Objective:To observe the clinical effect of modified Linggui Zhugan decoction(加味苓桂术甘汤)in the treatment of patients with prediabetes(spleen deficiency and phlegm-dampness syndrome)and its effect on the changes of blood glucose and blood lipid in patients.Methods:Sixty patients who met the clinical observation criteria were randomly divided into the treatment group and the control group,with 30 patients in each group.The patients in both groups were given basic treatment measures such as diabetes diet and appropriate exercise.The patients in the control group were given oral metformin sustained-release tablets,and the patients in the treatment group were treated with modified Linggui Zhugan decoction on the basis of the treatment in the control group.There was no statistical difference between the two groups in terms of gender,age,course of disease,body weight index,laboratory test indicators,TCM symptoms,(P>0.05),and the clinical observation course was 28 days.Results:After treatment,the total effective rate in the treatment group was 93.33%(28/30),and the total effective rate in the control group was 83.33%(25/30).There were statistically significant differences in the total efficacy and the amelioration of TCM of symptoms(P<0.05).Compared with the control group,the GLU,2 h PG,TC,LDI-C of patients in the treatment group were significantly reduced after treatment,and the difference was statistically significant(P<0.05).Conclusion:Patients with prediabetes have varying degrees of spleen Yang(阳)deficiency and phlegm dampness obstruction.The treatment effect of simple Western medicine is poor.The combination of Western medicine and TCM syndrome differentiation treatment can significantly improve the clinical efficacy and relieve the symptoms of patients,regulate blood glucose and lipids,etc.Modified Linggui Zhugan decoction is a traditional Chinese medicine prescription,which is better than the simple Western medicine treatment in treating prediabetes(spleen deficiency and phlegm dampness syndrome),an
