六味地黄汤加减治疗肾虚型头晕的临床经验

摘　　要：目的:随着经济的发展,社会的进步,不少人因为压力大、精神紧张、用脑过度或因失眠、耳石症、颈椎病等病症而出现头晕的表现,通常对症治疗能迅速缓解症状,但部分头晕患者常规用药疗效不佳,导致其工作及生活质量受影响,因而本研究着力于探索肾虚型头晕的治疗方法。方法:在辨证归属肾虚型头晕的情况下以经方六味地黄汤进行加减治疗。结果:经过2~3个疗程以六味地黄汤为基础方的加减治疗,患者头晕症状逐步消失,临床疗效突出,生活质量显著提高,其伴随症状如耳鸣、腰酸、下肢乏力也随之减轻或消失。结论:六味地黄汤由熟地黄、山药、山茱萸、牡丹皮、茯苓、泽泻组成,诸药合用共奏滋阴补肾兼清降虚火的功效。该方三补三泻,温中有凉、涩中有泻,具有滋补不留邪、祛邪不伤正的典型特点,可广泛运用于多种肾精不足、肝肾阴虚型病症,对肾虚型头晕治疗效果突出,且对肾虚型头晕的临床伴随症状起到良好的治疗效果,充分体现出六味地黄汤在补阴填精的同时抑制阳气过分亢盛,促使机体达到阴平阳秘、自稳平衡的状态。期待广大同道加强对经方治疗难治性病症的研究,以此进一步推动中医的发展,帮助更多患者解决顽疾困扰。Objective:With the development of economy and the progress of society,many people suffer from dizziness due to high pressure,mental tension,excessive use of the brain,or insomnia,otoliths,cervical spondylosis and other diseases.Usually,symptomatic treatment can quickly alleviate the dizziness symptom.However,some dizziness patients treated by conventional medication have poor efficacy,which affects their work and life quality.Therefore,this study focuses on exploring the treatment of dizziness of kidney deficiency type.Methods:In this case,based on syndrome differentiation of dizziness of kidney deficiency type,classic prescription Liuwei Dihuang decoction(六味地黄汤)was used.Results:After 2 to 3 courses of treatment with Modified Liuwei Dihuang decoction as the basic prescription,the dizziness symptoms of the patients gradually disappeared,the clinical effect was outstanding,the quality of life was significantly improved,and the accompanying symptoms such as tinnitus,lumbar acid,and weakness of the lower limbs were also alleviated or disappeared.Conclusion:Liuwei Dihuang decoction is composed of prepared Shudihuang(Rehmannia root),Shanyao(Rhizoma discoreae),Shanzhuyu(Cornus officinalis),Mudanpi(Peony bark),Fuling(Poria cocos),and Zexie(Alisma orientalis).The combination of these herbs has the effect of nourishing Yin(阴)and tonifying the kidney,as well as clearing and lowering deficiency fire.The prescription has the function of three reinforcing and three reducing,including cold in warm and reducing in astringent,which has the typical characteristics of nourishing without retaining evil and eliminating evil without harming the body.It can be widely used in various diseases of kidney essence deficiency,liver and kidney Yin deficiency,and has outstanding therapeutic effects on dizziness of kidney deficiency type.It also has good therapeutic effects on clinical accompanying symptoms of dizziness of kidney deficiency type,fully reflecting that Liuwei Dihuang decoction not only supplements Yin and fills essenc
