摘  要:目的比较瑞马唑仑与丙泊酚在无抽搐电休克治疗(MECT)中的应用效果。方法选取我院2023年3月-6月择期行MECT治疗的60例患者为研究对象,随机分为瑞马唑仑组(A组)和丙泊酚组(B组),每组30例。A组静注琥珀胆碱1.0 mg/kg、瑞马唑仑0.3 mg/kg,B组静注琥珀胆碱1.0 mg/kg、丙泊酚2.0 mg/kg。记录两组患者入室时(T0)、诱导后(T1)、电刺激时(T2)、电刺激后5 min(T3)、电刺激后10 min(T4)的平均动脉压(MAP)、心率(HR)、脉氧饱和度(SpO_(2))及呼吸恢复时间、停药后苏醒时间、麻醉诱导时间、抽搐电量、抽搐发作时间、不良反应发生情况。结果A组患者T1、T2、T3、T4时MAP、HR均高于B组(P<0.05),两组患者T0时MAP、HR比较,差异无统计学意义(P>0.05),两组患者各个时间点SpO_(2)比较,差异无统计学意义(P>0.05);A组呼吸恢复时间、停药后苏醒时间短于B组(P<0.05),而镇静药诱导时间、抽搐发作时间长于B组(P<0.05);A组注射痛发生率低于B组(P<0.05),两组恶心呕吐发生率比较,差异无统计学意义(P>0.05)。结论瑞马唑仑在MECT治疗中可使患者呼吸恢复时间与苏醒时间短于丙泊酚,抽搐发作时间长于丙泊酚,但对呼吸和循环的影响较小,不良反应发生率低,与丙泊酚相比具有一定的优势。Objective To compare the application effects of remazolam and propofol in modified electroconvulsive therapy(MECT).Methods Sixty patients who underwent MECT in our hospital from March to June 2023 were randomly divided into remazolam group(group A)and propofol group(group B),with 30 patients in each group.Group A was intravenously injected with 1.0 mg/kg succinylcholine and 0.3 mg/kg remazolam,and group B was intravenously injected with 1.0 mg/kg succinylcholine and 2.0 mg/kg propofol.The mean arterial pressure(MAP),heart rate(HR),pulse oxygen saturation(SpO_(2))at the time of entering the room(T0),after induction(T1),during electrical stimulation(T2),5 min after electrical stimulation(T3)and 10 min after electrical stimulation(T4),as well as respiratory recovery time,recovery time after drug withdrawal,anesthesia induction time,convulsion power,convulsion attack time and adverse reactions were recorded.Results MAP and HR at T1,T2,T3 and T4 in group A were higher than those in group B(P<0.05),but there was no significant difference in MAP and HR between the two groups at T0(P>0.05),there was no significant difference in SpO_(2) between the two groups at each time point(P>0.05).The respiratory recovery time and recovery time after drug withdrawal in group A were shorter than those in group B(P<0.05),while the sedative induction time and seizure time were longer than those in group B(P<0.05).The incidence of injection pain in group A was lower than that in group B(P<0.05).There was no significant difference in the incidence of nausea and vomiting between the two groups(P>0.05).Conclusion In the treatment of MECT,compared with propofol,remazolam can shorten the respiratory recovery time and recovery time of patients.Although it prolongs the seizure time,it has little effect on respiration and circulation,and the incidence of adverse reactions is low,which has certain advantages.
