通络益胃汤治疗慢性萎缩性胃炎的有效性及对复发率的影响

摘　　要：目的分析通络益胃汤治疗慢性萎缩性胃炎的有效性及对复发率的影响。方法选取2020年1月-2022年1月在我院诊治的66例慢性萎缩性胃炎患者为研究对象,采用随机数字表法分为对照组和观察组,各33例。对照组采用常规西药治疗,观察组在对照组基础上给予通络益胃汤治疗,比较两组临床疗效、中医证候(胃痛、嗳气、胃胀)评分、内镜病理积分、不良反应发生率及复发率。结果观察组治疗总有效率(93.94%)高于对照组(84.85%)(P<0.05);两组治疗后胃痛、嗳气、胃胀评分均低于治疗前,且观察组低于对照组(P<0.05);两组治疗后内镜病理积分均低于治疗前,且观察组低于对照组(P<0.05);观察组不良反应发生率与对照组比较,差异无统计学意义(P>0.05);随访1个月,观察组复发率(9.09%)低于对照组(21.21%)(P<0.05)。结论通络益胃汤治疗慢性萎缩性胃炎具有较显著的有效性,可减轻中医证候,降低内镜病理积分和复发率,且不会增加不良反应,值得临床加以应用。Objective To analyze the effect of Tongluo Yiwei decoction on the efficacy and recurrence rate of chronic atrophic gastritis.Methods A total of 66 patients with chronic atrophic gastritis diagnosed and treated in our hospital from January 2020 to January 2022 were selected as the research objects.They were divided into control group and observation group by random number table method,with 33 patients in each group.The control group was treated with conventional western medicine,and the observation group was treated with Tongluo Yiwei decoction on the basis of the control group.The clinical efficacy,TCM syndrome(stomach pain,belching,stomach distension)score,endoscopic pathological score,incidence of adverse reactions and recurrence rate were compared between the two groups.Results The total effective rate of treatment in the observation group(93.94%)was higher than that in the control group(84.85%)(P<0.05).After treatment,the scores of stomachache,belching and bloating in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).After treatment,the endoscopic pathological score of the two groups was lower than that before treatment,and that of the observation group was lower than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the observation group and the control group(P>0.05).After 1 month of follow-up,the recurrence rate of the observation group(9.09%)was lower than that of the control group(21.21%)(P<0.05).Conclusion Tongluo Yiwei decoction is effective in the treatment of chronic atrophic gastritis,which can reduce TCM syndromes,endoscopic pathological score and recurrence rate,and will not increase adverse reactions.It is worthy of clinical application.
