摘  要:目的:利用MRI定量磁化率成像(QSM),分析帕金森病(PD)患者各基底核团磁敏感值的变化特点。方法:选取2021年1月—2023年12月在宁夏回族自治区人民医院确诊的PD患者15例为PD组,选择同期性别及年龄匹配的健康志愿者20例作为健康对照组。所有患者均进行QSM,在QSM图像上测量所有受试者双侧尾状核头、苍白球、壳核、红核、黑质及双侧小脑齿状核的磁敏感值。采用独立t检验比较PD组与健康对照组各灰质核团磁敏感值的差异。结果:PD组患者与健康对照组的双侧尾状核头、双侧苍白球、双侧壳核、左侧齿状核的磁敏感对比,差异均无统计学意义(P>0.05),PD组患者双侧黑质、红核的磁敏感值较健康对照组升高,差异均有统计学意义(P<0.05)。PD组患者的右侧小脑齿状核磁敏感值较对侧升高,差异有统计学意义(P<0.05)。结论:QSM成像能够作为一种无创方法对PD患者脑组织铁含量进行定量测量。PD患者红核、黑质区域的磁敏感值较正常对照组明显升高。QSM为PD患者的早期诊断提供了可靠的影像学依据,对患者病情评估、治疗方案的选择具有重要意义,在临床中具有重要的临床应用价值。Objective To use MRI quantitative susceptibility mapping(QSM)to analyze the changes in magnetic sensitivity values of various basal nuclei in patients with Parkinson's disease(PD).Methods Fifteen PD patients diagnosed in People's Hospital of Ningxia Hui Autonomous Region were selected,and 20 healthy volunteers matched in gender and age during the same period were selected as the control group.All patients underwent QSM,and the magnetic sensitivity values of the bilateral head of the caudate nucleus,globus pallidus,putamen,red nucleus,substantia nigra,and bilateral cerebellar dentate nucleus were measured on QSM.The independent sample t-test was used to compare the differences in magnetic sensitivity values of gray matter nuclei between the PD group and the healthy control group.Results There was no significant difference in magnetic sensitivity between PD patients and the healthy control group in terms of bilateral caudate head,bilateral globus pallidus,bilateral putamen,and left dentate nucleus(P>0.05).The magnetic sensitivity values of the bilateral substantia nigra and nucleus rubra in PD patients were higher than those in the healthy control group,and the differences were statistically significant(P<0.05).The sensitivity value of right cerebellar dentate magnetic resonance imaging in Parkinson's patients was higher than that in the contralateral side,and the difference was statistically significant(P<0.05).Conclusion QSM can serve as a non-invasive method for quantitative measurement of iron content in brain tissue of PD patients.The magnetic sensitivity values of the red nucleus and substantia nigra in PD patients were significantly higher than those in the normal control group.QSM provides a reliable imaging basis for the early diagnosis of PD patients,which is of great significance for evaluating the patient's condition and selecting treatment plans.It has important clinical application value in clinical practice.
