摘　　要：目的 探讨个性化护理策略在老年危重症患者人工气道管理中的应用效果。方法 选取2022年7—12月三明市第二医院重症医学科收治的急性呼吸衰竭和慢性呼吸衰竭加重期老年危重症患者140例为研究对象,采用随机数表法分为观察组和对照组,每组各70例,对照组采用常规护理方法,观察组采用个性化护理策略。比较两组患者人工气道管理效果。结果 观察组患者痰液引起的刺激性咳嗽、胃内容物反流及气道黏膜损伤发生率均低于对照组,差异有统计学意义(P <0.05);观察组患者人工气道堵管以及医院获得性肺部感染发生率均低于对照组,吸痰间隔时间长于对照组,差异有统计学意义(P <0.05)。结论 个性化护理策略在老年危重症患者气道管理中的效果显著,可降低并发症发生率,提高护理质量,值得临床应用。Objective To investigate the application effect of individualized nursing strategy in artificial airway management of elderly patients with critical illness.Methods A total of 140 elderly patients with critical illness with acute respiratory failure and exacerbation of chronic respiratory failure admitted to the Department of Critical Care Medicine of Sanming Second Hospital from July to December 2022 were selected as the research objects,and they were divided into the observation group and the control group by random number table method,with 70 cases in each group.The control group was treated with conventional nursing methods,while the observation group was treated with individualized nursing strategy.The effect of artificial airway management in the two groups of elderly patients with critical illness was compared.Results The incidences of irritating cough caused by phlegm,gastric contents regurgitation and airway mucosal injury of patients in the observation group were all lower than those of patients in the control group,with statistically significant differences(P<0.05).The incidences of artificial airway obstruction and hospital-acquired pulmonary infection of patients in the observation group were all lower than those in the control group,and the interval of sputum aspiration was longer than that in the control group,with statistically significant differences(P<0.05).Conclusion Individualized nursing strategy is effective in airway management of elderly patients with critical illness,which can reduce the incidence of complications and improve the quality of nursing.Therefore,it is worthy of clinical application.
