摘  要:双胎输血综合征(TTTS)是双胎妊娠中一种严重的并发症,发病率和病死率均较高。其发病机制为单绒毛膜双羊膜囊双胎胎盘中血管发生异常吻合,导致胎儿血流灌注产生差异。目前产前主要通过超声检测双胎的二维图像并结合胎儿的血流频谱诊断TTTS。脐动脉是胎儿时期重要的血管,其频谱多普勒可直接反映胎儿血流灌注状况,在TTTS诊断、分期及胎儿激光术后结局预测等方面均发挥重要作用。双胎间脐动脉的频谱多普勒参数的差值可及时反映双胎灌注差异,全面了解脐动脉频谱多普勒在TTTS诊疗中的作用,可以为疾病的治疗提供新思路。Twin-twin transfusion syndrome(TTTS)is a serious complication of twin pregnancy,with both high incidence and mortality.The pathogenesis is abnormal anastomosis of blood vessels in the monochorionic diamoniotous twin placenta,which lead to differences in fetal blood flow perfusion.At present,prenatal diagnosis of TTTS is mainly achieved through ultrasound detection of two-dimensional images of twins and combined with fetal blood flow spectrum.The umbilical artery is an important blood vessel in the fetal period,and its spectral Doppler can directly reflect the fetal blood flow perfusion status,which plays an important role in TTTS diagnosis,staging,and prediction of fetal laser surgery outcomes.The difference in spectral Doppler parameters of the umbilical artery between twins can reflect the difference in perfusion of the twins in a timely manner.A comprehensive understanding of the role of umbilical artery spectral Doppler in the diagnosis and treatment of TTTS can provide new ideas for the treatment of related diseases.
