不同骨移植材料治疗跟骨骨囊肿的临床研究

摘　　要：目的:探讨不同骨移植材料对跟骨骨囊肿治疗的影响。方法:选取2011年6月—2019年8月样本医院收治的35例应用刮除植骨术治疗的跟骨骨囊肿患者作为研究对象。根据植入材料的不同分为A、B两组,A组(18例)使用同种异体骨-鑫康晨移植,B组(17例)使用自体髂骨结合同种异体骨(鑫康晨)移植(比例1∶2)。记录两组患者围术期指标、血清指标及炎性因子、术后并发症的发生情况、术后3个月及术后4个月临床治愈率、术前与术后5个月AOFAS踝-后足评分。结果:A组手术时间、术中出血量均少于B组,差异有统计学意义(t=9.204、18.470,P<0.05);术后3个月,两组患者间血清碱性磷酸酶(ALP)及骨钙素(BGP)比较,差异有统计学意义(t=7.036、5.481,P<0.05);术后1周,两组患者间C反应蛋白(CRP)比较,差异有统计学意义(t=15.022,P<0.05);术后1周,A组CRP高于B组,术后3个月临床治愈率,A组优于B组;两组患者术后并发症发生率比较,差异有统计学意义(P<0.05);两组患者术后5个月AOFAS踝-后足评分较术前比较有所改善,差异有统计学意义(t=1.386、0.453,P<0.05)。结论:跟骨骨囊肿刮除后应用自体骨结合异体骨移植与单纯同种异体骨移植相比,混合骨移植对早期愈合率有影响,而单纯同种异体骨移植影响机体炎症指标的恢复程度,但单纯同种异体骨移植具有手术时间短、创伤小、术后并发症少等优势。Objective:To explore the effect of different bone graft materials on the treatment of calcaneal bone cyst.Methods:The clinical data of 35 cases of calcaneal bone cyst treated by curettage and bone grafting were analyzed from June 2011 to August 2019.According to different implant materials,they were divided into group A and group B.18 cases in group A were treated with allogeneic transplantation(Xin Kangchen).17 cases in group B were transplanted with autologous iliac bone and allogeneic bone(Xin Kangchen),and the ratio was 1∶2.The perioperative indexes,serum indicators and inflammatory factor,the incidence of postoperative complications,the clinical cure rate 3 months and 4 months after operation,and AOFAS ankle hindfoot score preoperation and 5 months after operation were recorded.Results:The operation time and intraoperative bleeding in group A were lower or less than those of group B(t=9.204,18.470;P<0.05).Serum alkaline phosphatase(ALP)and osteocalcin(BGP)in group A were better than those in group B 3 months after operation(t=7.036,5.481;P<0.05).There was statistically significant difference in CRP between the two groups 1 month after operation(t=15.022,P<0.05).CRP of group A was higher than group B 1 week after operation,recovery rate of group A was superior to group B 3 months after operation,and there was statistically significant difference in the compliance rate of two groups(P<0.05).The AOFAS ankle-hindfoot score 5 months after operation of the two groups improved compared with those before operation(t=1.386,0.453;P<0.05).Conclusion:Autogenous bone combined with allogeneic bone transplantation has an impact on the early healing rate compared with simple allogeneic bone transplantation after curettage of calcaneal bone cyst,while simple allogeneic bone transplantation influences the recovery of inflammatory indexes.But it has the advantages of shorter operation time,lesser trauma and lesser postoperative complications.
