摘 要:目的探讨T_(2)^(*)mapping定量评价新兵奔袭集训前后膝关节软骨及软骨下骨变化的价值。方法对20例新兵进行右膝关节MRI扫描3次:奔袭集训前、集训1周后及休整1个月后。将膝关节软骨分为股骨外侧(LF)、股骨内侧(MF)、胫骨外侧(LT)、胫骨内侧(MT)、髌骨软骨(PC)及滑车软骨(TC)6个区域。以半月板后角为界,将LF和MF分为股骨髁中央承重区(cLF/cMF)和后部非承重区(pLF/pMF);以软骨厚度1/2为界分为浅层区域(SZ)软骨和深层区域(DZ)软骨。将软骨下骨划分紧贴关节软骨5mm以内为浅层骨质(SB),距离关节软骨6~10mm为深层骨质(DB),通过感兴趣区(ROI)分析评估软骨及软骨下骨各区域T_(2)^(*)值。利用单因素方差分析比较T_(2)^(*)值变化,组间比较采用LSD检验法。结果集训1周后MT-SZ、cMF-SZ、PC-SZ、TC-SZ均明显高于集训前(P<0.05);休整1个月后,软骨区域的T_(2)^(*)值与集训前相比,差异无统计学意义(P>0.05),呈“先升后降”的变化趋势。集训前与集训1周后相比,膝关节软骨下骨T_(2)^(*)值差异均无统计学意义(P>0.05),休整1个月后除cLF-DB、pLF-DB、滑车软骨深层骨质(TC-DB)外,余膝关节软骨下骨T_(2)^(*)值集训前、集训1周后与休整1个月后相比均上升,差异均有统计学意义(P<0.05)。结论T_(2)^(*)mapping可显示新兵奔袭集训后关节软骨及软骨下骨超微结构和生化成分的变化,发现早期损伤并能定量无创性评估。Objective To explore the value of T_(2)^(*)mapping in quantitatively evaluate changes in knee joint cartilage and subchondral bone of new recruits before and after intensive training.Methods MRI scans of the right knee joint were performed three times on 20 new recruits:before intensive training,after one weck of intensive training,and after one month of rest.The knee joint cartilage was divided into six regions:lateral femur(LF),medial femur(MF),lateral tibia(LT),medial tibia(MT),patella cartilage(PC),and trochlea cartilage(TC).Using the posterior angle of the meniscus as a boundary,LF and MF were divided into the cLF/cMF and pLF/pMF.Divided into superficial zone(SZ)cartilage and deep zone(DZ)cartilage based on a thickness of 1/2 of the cartilage.The subchondral bone was divided into superficial bone(SB)within 5 mm of the joint cartilage,and deep bone(DB)within 6-10 mm of the joint cartilage.The T_(2)^(*)values of each region of cartilage and subchondral bone were evaluated through region of interest(ROl)analysis.Single factor analysis of variance was used to compare the changes in T_(2)^(*)values.The LSD test method was used for inter-group comparison.Results After one week of intensive training,MT-SZ,cMF-SZ,PC-SZ,TC-SZ were significantly higher than before intensive training(P<0.05).After one month of rest,there was no statistically significant difference in the T_(2)^(*)value of the cartilage area compared with before intensive training(P>0.05).There was a trend of"rising first and then falling".There was no statistically significant difference in the T_(2)^(*)value of subchondral bone of the knee joint before intensive training and after one week of intensive training(P>0.05).Compared with after one month of rest,except for cLF-DB,pLF-DB,trochlea cartilage-deep bone(TC-DB),the T_(2)^(*)value of the subchondral bone of the remaining knee joint increased before intensive training and after one week of intensive training,with statistically significant dfferences(P<0.05).Conclusion T_(2)^(*)mapping can display t
