学龄前儿童胸部CT检查过程中陪护位置的选择

摘　　要：目的评估学龄前儿童接受胸部CT检查过程中,陪护者在不同位置所接受的辐射剂量,并探寻最佳陪护位置。方法采用2种扫描方案,3岁以下儿童扫描管电压为80kV,3岁以上儿童扫描管电压为100kV,采用自动管电流调制技术。在3个常见陪护位置分别于2种扫描方案下各测量10例,共60例。对比3个常见陪护位置的辐射剂量并进行统计学分析。结果儿童胸部CT扫描时,检查机房内3个陪护位置的辐射剂量均有差异(P<0.01)。管电压100kV时,剂量从大到小依次为:机架入口处(9.28±2.37)μSv,机架背侧开口处(6.03±0.70)μSv,距离机架入口1m处(0.99±0.16)μSv;管电压80kV时剂量从大到小依次为:机架入口处(5.25±0.82)μSv,机架背侧开口处(3.36±0.41)μSv,距离机架入口1m处(0.70±0.06)μSv。结论学龄前儿童胸部CT检查过程中,对于配合度高的患儿,监护人可选择距离机架1m处进行陪护;需要近距离固定安抚的患儿,监护人首选机架背侧开口处陪护。Objective To evaluate the radiation doses received by guardians at different positions when accompanying preschool children undergoing chest CT scans and to explore the optimal accompanying position.Methods Two scanning protocols were used,with 80 kV tube voltage for children under 3 years old and 10o kV for children above 3 years old,using automatic tube current modulation technique.Measurements were taken at three common accompanying positions for each scanning protocol,with 10 cases measured in each position,totaling 60 cases.Radiation doses at the three common accompanying positions were compared and statistically analyzed.Results There were significant differences in the radiation doses at three accompanying positions inside the examination room during children chest CT scans(P<0.01).When the tube voltage was 1oo kV,the order of doses from highest to lowest was:at the entrance of the gantry(9.28±2.37)μSv,at the back opening of the gantry(6.03±0.70)μSv,and at a distance of 1 m from the gantry entrance(0.99±0.16)μSv;when the tube voltage was 80 kV,the order of doses from highest to lowest was:at the entrance of the gantry(5.25±0.82)μSv,at the back opening of the gantry(3.36±0.41)μSv,and at a distance of 1 m from the gantry entrance(0.70±o.06)μSv.Conclusion During chest CT examinations of preschool children,for well-cooperative children,guardians may choose to accompany them at a distance of 1 m from the gantry;for children requiring close fixation and reassurance,guardians are recommended to accompany them at the back opening of the gantry.
