膏方治疗心系疾病的辨治思路和处方规律研究

摘　　要：目的:通过对大量医案进行深入的数据挖掘及规律分析,为研究膏方治疗心系疾病提供更多依据,为临床提供参考借鉴。方法:对相关医案进行数据分析,探索医案中膏方治疗心系疾病的辨治思路及处方用药规律。结果:收集符合标准医案90则,涉及中药346味。发现心系疾病常见高频症状有心悸、头晕、神疲乏力等;药物以甘温药物为主;归经主要为肝、肾、脾、肺、心;主要证候为肝肾亏虚、痰瘀交阻、气滞血瘀等证候;主要功效为滋水涵木、祛痰化瘀、补益肝肾等。结论:历代医家运用膏方治疗心系疾病的辨治思路是补、活、清、和,通过调和机体的阴阳气血以及脏腑机能以达到动态平衡的目的;发现历代医家运用膏方治疗心系疾病的处方规律为药物多用补益气血类、滋阴补阳类、活血化瘀类等;多用药对加减,其中多以四物汤、三才汤、百合地黄汤、炙甘草汤等经典方剂化裁。Objective:By collecting a large number of medical cases and conducting in-depth data mining and analysis to provide foundation of ointment prescription for the treatment of heart diseases and offers valuable references for clinical practice.Methods:Manual retrieval was conducted to analyze the relevant medical record data and explore the differentiation and treatment ideas,and prescription medication rules for ointment prescriptions for the treatment of heart diseases.Results:Ninety medical records meeting the standard were collected,involving 346 drugs.Popular symptoms of heart disease included palpitations,dizziness,and fatigue.The medication primarily drugs with sweet and warming properties.The primary channel tropism included the liver,kidneys,spleen,lungs,and heart.The main syndromes were liver and kidney deficiency,phlegm and blood stasis intertwined,and Qi stagnation and blood stasis.The main functions were nourishing water,moistening wood,dissolving phlegm and blood stasis,and nourishing liver and kidneys.Conclusion:This study found that the TCM physicians in the past dynasties usually used ointment prescriptions to treat heart diseases by invigorating,clearing,and harmonizing methods,aiming to achieve dynamic balance through harmonizing the Yin,Yang,Oi,blood of the body as well as the functions of zang-fu organs.The results revealed that the prescription rules for ointment used by physicians in past dynasties to treat heart diseases including enriching Qi and blood,nourishing Yin and Yang,promoting blood circulation,and removing stasis.Many medicines were used for addition or subtraction,including Siwu Soup,San Cai Soup,Lily Ground Yellow Soup,Prepared Glycyrrhiza Soup,and other classic prescriptions.
