摘 要:目的探讨医共体抗菌药物管理(antimicrobial stewardship,AMS)模式在钬激光碎石术围手术期抗菌药物干预中的应用效果。方法选择2022年6月—2023年6月于淳安县第一人民医院行钬激光碎石术治疗的患者70例,采用随机数字表法将其分为对照组和干预组各35例。对照组给予常规化管理;干预组在对照组基础上采用AMS模式,比较两组患者抗菌药物的使用情况。结果干预组抗菌药物适应证用药、用药时机、药物、疗程、给药途径、用法用量、联合用药、溶媒及比例选择的合理性均高于对照组,差异有统计学意义(P<0.05)。干预组药品保管、处方开具、抗菌药物应用及药学服务评分均高于对照组,差异有统计学意义(P<0.05)。干预组抗菌药物使用率、抗菌药物使用强度、住院用药率均低于对照组,差异有统计学意义(P<0.05)。干预组管理满意度高于对照组,差异有统计学意义(P<0.05)。结论AMS模式用于围手术期抗菌药物干预中,有助于规范抗菌药物的合理使用,提高抗菌药物管理质量及患者管理满意度。Objective To explore the application effect of antimicrobial stewardship(AMS)model in perioperative antimicrobial intervention of holmium laser lithotripsy in medical community.Methods A total of 70 patients from June 2022 to June 2023 admitted to Chun'an County First People's Hospital undergoing holmium laser lithotripsy were selected and divided into control group and intervention group by random number table method,with 35 cases in each group.The control group was given the routine management,while the intervention group was given the AMS model based on the control group.The use of antibiotics was compared between two groups.Results The rationality of indications of antimicrobial drugs,timing,drugs,therapy course,route of administration,usage and dosage,combination of drugs,solvent and proportion selection of intervention group were significantly higher compared to the control group(P<0.05).The pharmaceutical care scores of drug storage,prescription issuance and antibiotic use of intervention group were significantly higher compared to the control group(P<0.05).The use rate and intensity of antibiotics and the rate of hospitalization in the intervention group were significantly lower compared to the control group(P<0.05).The management satisfaction of intervention group was significantly higher compared to the control group(P<0.05).Conclusion The AMS model used in perioperative antimicrobial intervention could help to standardize the rational use of antibiotics,improve the quality of antimicrobial management,and enhance patient satisfaction.
