NLR MLR和PLR对小儿IgM阳性支原体肺炎的诊断效能

摘  要:目的探讨中性粒细胞/淋巴细胞比率(neutrophil lympho cyte ratio,NLR)、单核细胞/淋巴细胞比率(monocytelymphocyteratio,MLR)和血小板/淋巴细胞比率(platelebletlymphocyteratio,PLR)对IgM阳性支原体肺炎的诊断效能。方法收集2022年1月—2023年12月盘锦市中心医院收治的小儿支原体肺炎病例80例作为研究组;另选同期健康体检小儿80例作为对照组。比较两组小儿各项血液成分水平;计算NLR、MLR和PLR并分析组间差异;通过受试者操作特征曲线(receiver operating characteristic,ROC)计算曲线下面积(area under the curve,AUC),评价其对支原体感染的诊断效能。结果两组小儿白细胞、中性粒细胞、单核细胞、淋巴细胞差异有统计学意义(P<0.05)。两组小儿NLR、MLR和PLR差异有统计学意义(P<0.05)。ROC曲线分析显示,NLR、MLR和PLR的AUC分别为0.847、0.764和0.861。结论相比于传统的指标,NLR、MLR和PLR在诊断近期感染的支原体肺炎方面,具有较高的诊断效能,可作为早期鉴别致病源的有效方法。Objective To explore the diagnostic efficacy of neutrophil/lymphocyte ratio(NLR),monocyte/lymphocyte ratio(MLR),and platelet/lymphocyte ratio(PLR)in IgM positive mycoplasma pneumonia.Methods A total of 80 cases of pediatric mycoplasma pneumonia admitted to Panjin Central Hospital from January 2022 to December 2023 were collected as the study group.A total of 80 children who underwent health examinations during the same period were selected as the control group.The levels of various blood components were compared between two groups.NLR,MLR,and PLR were compared between two groups,and the area under the receiver operating characteristic curve(AUC)was calculated to evaluate their diagnostic efficacy for mycoplasma infection.Results There were significant differences in white blood cells,neutrophils,monocytes,and lymphocytes between the two groups(P<0.05).NLR,MLR,and PLR were significantly different between the two groups(P<0.05).The AUC of NLR,MLR,and PLR were 0.847,0.764,and 0.861.Conclusion Compared to traditional indicators,NLR,MLR,and PLR have higher diagnostic efficacy in recently infected mycoplasma pneumonia,which could be an effective method for early identification of pathogens.
