针刺联合神经康复治疗中重度周围性面瘫的疗效观察

摘　　要：目的 研究针刺配合康复治疗中重度周围性面瘫的疗效。方法 选取子长市人民医院2015年1月1日至2023年12月31日170例中重度面瘫患者,随机分为观察组和对照组,每组85例。对照组给于针刺治疗,西药给予醋酸泼尼松片和甲钴胺片,观察组在对照组基础上联合康复治疗。比较两组患者治疗前后多伦多(Sunnybrook)面神经评定系统评分、面神经功能(House-Brackmann,H-B)分级及临床疗效。结果 治疗前两组患者的Sunnybrook评分及H-B面神经功能分级比较差异无统计学意义(P>0.05);治疗后两组患者的Sunnybrook评分均明显高于治疗前,且观察组Sunnybrook评分明显高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者的H-B面神经功能分级均明显低于治疗前,且观察组明显低于对照组,差异有统计学意义(P<0.05)。观察组总有效率为94.2%,显著高于对照组的83.6%,差异有统计学意义(P<0.05)。结论 针刺联合康复治疗能有效改善中重度周围性面瘫患者的面部功能,促进神经恢复,缓解面部临床症状,提高临床治疗有效率。Objective To study of the efficacy of acupuncture with rehabilitation in the treatment of moderate-tosevere peripheral facial paralysis.Methods 170 patients with moderate-to-severe facial paralysis were selected from January 1,2015 to December 31,2023 from the People's Hospital of Zichang City,with 85 cases in each group.The control group was given acupuncture treatment,and western medicines were given with prednisone acetate tablets and mecobalamin tablets,while the observation group was given with combined rehabilitation treatment on the basis of the control group.The two groups were compared in terms of the score of Toronto(Sunnybrook) Facial Nerve Assessment System,House-Brackmann(H-B) and clinical efficacy before and after treatment.Results The Sunnybrook score and H-B facial nerve function grading of the two groups were not statistically significant before treatment(P>0.05).Sunnybrook scores were significantly higher in both groups after treatment than before treatment,and the Sunnybrook score of the observation group was significantly higher than that of the control group,with a statistically significant difference(P<0.05).The H-B facial nerve function classification was significantly lower in both groups after treatment than before treatment,and the observation group was significantly lower than the control group,with a statistically significant difference(P<0.05).The total effective rate of the observation group was 94.2%,which was significantly higher than that of the control group(83.6%),and the difference was statistically significant(P<0.05).Conclusion Acupuncture combined with rehabilitation therapy effectively improve facial function,promote nerve recovery,alleviate facial clinical symptoms,and improve clinical treatment efficiency in patients with moderate-to-severe peripheral facial paralysis.
