联合生物反馈电刺激疗法治疗产后女性盆底功能障碍性疾病疗效的Meta分析

摘　　要：目的评价联合生物反馈电刺激疗法治疗产后女性盆底功能障碍性疾病(pelvic floor dysfunction disease,PFD)的临床价值。方法遵循PRISMA声明,通过检索2015年1月至2021年3月间在Cochrane Library、PubMed、Embase、Web of science、中国知网(CNKI)、万方数字知识服务平台、中国生物医学文献数据库(CBM)等数据库中已发表的选用联合生物反馈电刺激疗法治疗产后女性PFD的随机对照临床试验相关文献,以单纯盆底肌肉训练为对照组,由2名研究者独立对所获得的文献进行筛选、质量评价及数据提取,进行Meta分析。结果共检索文献455篇,最终纳入21篇文献进行Meta分析。观察组Ⅰ类肌肌力≥Ⅳ级、Ⅱ类肌肌力≥Ⅳ级、综合肌力≥Ⅳ级的占比优于对照组,差异均具有统计学意义(P<0.05)。观察组盆底器官脱垂程度改善效果优于对照组(P<0.01)。观察组尿失禁疗效显著高于对照组(P<0.01)。观察组PDF疗效显著高于对照组(P<0.05)。结论相比单纯盆底肌肉训练,联合生物反馈电刺激疗法治疗盆底功能障碍性疾病疗效显著,可明显提升盆底肌肌力。Objective To evaluate the clinical value of combined biofeedback electrical stimulation therapy in the treatment of postpartum pelvic floor dysfunction disease(PFD).Methods Following the PRISMA statement,the Cochrane Library,PubMed,Embase,and Web of Science,China National Knowledge Network(CNKI),Wanfang Digital Knowledge Service Platform,China Biomedical Literature Database(CBM) and other databases were searched from January 2015 to March 2021.The published clinical randomized controlled trials were selected combined biofeedback electrical stimulation therapy for postpartum women with PFD;Simple pelvic floor muscle training was used as the control group.Two researchers independently screened the obtained literature,evaluated its quality and extracted data for meta-analysis.Results A total of 455 literatures were searched,and 21 literatures were finally included for meta-analysis.The proportion of class Ⅰ muscle strength ≥ class Ⅳ,class Ⅱ muscle strength ≥ class Ⅳ and comprehensive muscle strength ≥ class Ⅳ in the observation group was better than that in the control group(P<0.05).The improvement effect of pelvic floor organ prolapse in the observation group was better than that in the control group(P<0.01).The curative effect of urinary incontinence in the observation group was significantly higher than that in the control group(P<0.01).The curative effect of PDF in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Compared with simple pelvic floor muscle training,combined with biofeedback electrical stimulation therapy is effective in the treatment of pelvic floor dysfunction diseases,which can significantly improve the muscle strength of pelvic floor muscle.
